The condition survey report shall comprise the output of visual inspection and NDT results. Major sections to be covered in the report shall be: (i) Identification of the cause of the distress. (ii) Systematic documentation of all observations during visual inspection, including distress maps. (iii) Systematic documentation of all test results obtained from NDTs. (iv) Determination of concrete properties based on NDTs (v) Requirement of propping of structural members to relieve part/full load acting on the member. (vi) Requirement of immediate evacuation and safety measures to prevent any immediate major mishap. (vii) Recommendations on repair/retrofit strategy with due consideration to available space and accessibility for ongoing activities. (viii) Requirements on safety measures to be adopted during execution of repair/retrofit jobs. (ix) Limitations/assumptions used. (x) Any other recommendation.
