Distress symptoms will be marked on floor plan as per notation used or as directed by Project Engineer and prepare data sheets mentioning exact location of each crack, height, and width of crack and Preparation and submission of two copies of draft report as specified followed by five copies of final report along with a soft copy of the same on diskette covering distress mapping Analysis observations and conclusions along with photos etc., Complete as directed by Engineer-in-Charge. Note: Condition Survey of the area under consideration shall be carried out, which includes a) Visual Inspection: Grouping of structural members should be done during visual inspection based on environmental exposure conditions e.g. external and internal members. Also, identification of Corrosion prone locations as per Visual Inspection. Follow the notations and typical worksheet for recording distress during visual inspection and damage classes based on visual inspection as per CPWD handbook on repair. The condition survey report shall comprise the output of visual inspection and NDT results. Major sections to be covered in the report shall be: (i) Identification of the cause of the distress. (ii) Systematic documentation of all observations during visual inspection, including distress maps. (iii) Systematic documentation of all test results obtained from NDTs. (iv) Determination of concrete properties based on NDTs (v) Requirement of propping of structural members to relieve part/full load acting on the member. (vi) Requirement of immediate evacuation and safety measures to prevent any immediate major mishap. (vii) Recommendations on repair/retrofit strategy with due consideration to available space and accessibility for ongoing activities. (viii) Requirements on safety measures to be adopted during execution of repair/retrofit jobs. (ix) Limitations/assumptions used. (x) Any other recommendation.
