Description Procurement Of One General Pest Control Services For The Intellectual Property Office Of The Philippines 1 Lot I. Qualifications Of The Service Provider • Competent And Licensed Contractor To Provide General Comprehensive Pest Control Services Specially Trained Technician And Certified By The Fertilizer And Pesticide Authority (fpa) And/or Of The Food And Drugs Administration (fda) • Certified For Satisfactory Service From Former/current Clients (3 Private Companies Or Government Agencies) • Must Have Impeccable Tracyecord In Pest Control Industry. • Must Be Iso 9001 :2015 Certified Ii. Scope Of Services The Service Provider Shall Provide Pest Control Services And Supplies For The Ipophl And Shall Undertake The Following: 1. General Pest Control Services • The Service Provider Shall Identify, Inspect, Asses And Treat All Known And Accessible Harborage Areas Within The Ipophl Premises. Elimination And Control Of Cockroaches (german Cockroaches), And Other Crawling Insects; Houseflies, Mosquitoes And Other Flying Insects; And Rodents • The Service Provider Shall Submit Pest Control Plan • The Service Provider Shall Treat The 5091 Square Meters Area Of The Ipophl In Intellectual Property Center, Mckinley Hill, Taguig City That Will Be Moved To Savya Financial Center, Arca South, Taguig City, Covering An Area Of 6002 Square Meters In Total Without Additional Cost To Ipophl. 2. Treatment Method The Following Methods Shall Be Implemented: A. Mist Spraying - Applicable In The Elimination Of Flying Insects B. Residual (by Applying Pelleted Roach Bait, Powder, And Application Of Insect Growth Regulator Concentrate) / Surface Spraying - Applicable In The Elimination Of Crawling And Flying Insects That Come Into Contact Of The Treated Surfaces C. Setting Up Of Mechanical Traps, Glues, And Installation Of Encapsulated Zinc Phosphide Concentrate - Applicable In The Elimination Of Rodents; Inclusive Of Baits 3. Chemicals, Equipment And Manpower For Pest Control A. The Service Provider Shall Provide And Use The Necessary Chemicals And Equipment That Are Safe To Humans, Environr1j.ent-friendly, Non-damaging To Ipophl Documents, Property & Equipment And Ensure Effective Treatment And Elimination Of The Identified Pests. B. The Chemical Products Shall Be Drawn Only From Those Chemicals Certified By The Fertilizer And Pesticide Authority Of The Philippines (fpa) And/or The Food And Drugs Administration (fda) C. The Service Provider Shall Provide A Team Consisting Of At Least Two (2) Service Technicians During The Conduct Of The General Pest Control Service. D. The Service Provider Shall Deploy Service Supervisor And Quality Assurance Officer Monthly/after Each Treatment To Conduct Quality Inspection And Monitoring To Check Quality Of Treatment Being Rendered. 4. Documentation • The Service Provider Shall Submit Service Report (after The Treatment) And After Service Inspection Report (after The Inspection Or Day/s After The Treatment) Indicating The Type Of Treatment Done With Recommendations To Help Control And Minimize Re-infestation In The Ipophl Premises After Every Treatment. Iii. Work Schedule The Service Provider Shall Conduct The General Pest Control Services Twice A Month Or At Least Every Fifteen (15) Days Cycle. Prior To The Conduct Of General Pest Control Service, The Service Provider Shall Submit The List Of Manpower, Equipment, And Chemicals To The Fmas-admin. Division At Least Five (5) Working Days Before The Conduct Of The Gpcs For The Preparation Of Work Permit And Gate Pass Iv. Approved Budget For The Contract ("abc") The Abc For This Procurement Is Five Hundred Thousand Pesos (php 500,000.00), Inclusive Of All Applicable Government Taxes And Other Miscellaneous Expenses Subject To The Usual Budgeting, Accounting, Auditing Rules And Regulations. V. Contract Duration The Contract Will Be Valid For A Period Of One (1) Year, Reckoning From The Signed Job Order.
