Description Terms Of Reference Procurement Of Preventive Maintenance Service Contract For The Hp Servers And Cisco Switches 1 Lot I Purpose Preventive Maintenance Service For The Hpe Servers And Cisco Switches Will Ensure That The Said Equipment Remains In Good Condition So As To Provide Continuous Uptime And Access To Data And Systems. Ii. Approved Budget For Contract (abc) The Abc Is Php700,000.00 For One Year Inclusive Of Government Taxes. Iii. Scope Of Work • Engagement Of A Service Provider For Preventive Maintenance Works. • Provide Service Level Agreement (sla) Which Defines Equipment Maintenance, Service Response Times, Etc. • Provide Replacement Of Equipment Or Parts Of The Same Or Alternative Specification Through Return Material Authorization (rma). • Provide Preventive Maintenance On A Quarterly Basis. Iv. Service Provider Qualifications • Service Providers Must Be In The Business For At Least Five (5) Years In Providing After-sales Services Of The Equipment To Be Maintained. • Service Providers Must Be Authorized By The Equipment Manufacturer To Support After-sales Services. A Photocopy Of The Latest Certification From The Equipment Manufacturer Should Be Submitted. • Service Providers Must Have At Least Two (2) Certified Engineers Who Are Regular And Locally Based Employees And Must Be Certified And Trained By The Hardware Manufacturer. A Photocopy Of The Certifications Valid For At Least Three (3) Years Should Be Submitted. A Certification From The Hr Or Admin Officer Of The Company Stating That These Employees Are Currently Employed To The Company Should Also Be Submitted. • Service Providers Must Have Support Services Available 24x7 Via Phone Or Email With A Response Time Of 2 To 4 Hours. Procedures On Support And Problem Escalation Should Be Submitted. • Service Providers Must Have At Least Two (2) Clients With The Last Two Years With Similar Contract Scope. V. Service Provider Deliverables 1. Corrective Maintenance A. Attend Service Calls Of Ipophl By Providing Immediate Telephone Support To Diagnose And Remedy Any Reported Technical Problem. B. Provide On-site Technical Support Engineers To Perform Fault Isolation And Resolution On The Reported Problem. C. Provide Necessary Spare Parts For The Purpose Of Maintaining The Equipment Listed In Annex "a". 2. Preventive Maintenance A. Perform Routine Preventive Maintenance Every Quarter With The Following Scope: 1. General Check-up Of The Equipment Or Hardware Covered Under The Preventive Maintenance. 2. General Check-up Of Any Program Or Software Used In The Operation Of Any Equipment Or Hardware. 3. Conduct A Complete Diagnostic Routine Within The System. 4. Physical Cleaning Of The Covered Devices. 3. Hardware Support A. In Case The Equipment Is Found Faulty And The Same Needs To Be Pulled Out By The Service Provider To Raise A Return Material Authorization (rma) Ticket To The Hardware Manufacturer, A Service Replacement Unit Of The Same Or An Alternative Specification As That Of The Malfunctioning Equipment Shall Be Provided While The Faulty Equipment Is Undergoing Rma. The Service Provider Shall Shoulder All The Costs Related To The Shipment Of The Equipment Subject To Rma. B.. The Service Provider Guarantees That It Will Be Returned And Re-installed Fit-for-use To Ipophl Within Six (6) Weeks, Starting From The Date The Said Equipment Was Pulled Out. Should The Service Provider Fail To Return Or Re-install The Equipment Within The Said Period, The Service Unit Provided By The Service Provider Shall Become The Property Of The Ipophl And The Pulled-out Defective Equipment The Property Of The Service Provider, Provided That The Service Unit Shall Have All The Warranties Of A Brand New Unit. 4. Problem Reporting A. Provision Of Phone Support 24x7 Mondays Through Sundays, Including Holidays, With A 30-minute Response Time Upon Receipt Of The Call By The Service Provider Helpdesk. B. Provision Of On-site Support Within 2-4 Hours Response Time. Vi. Contract Period The Contract Term Is One Year From Receipt Of The Notice To Proceed. Vii. Payment Terms Quarterly Payment Shall Be Made Upon Completion And Submission Of A Report Duly Signed And Certified By The Mis Director After The Scheduled Quarterly Preventive Maintenance.
