Within Your Email Please Confirm Names And Email Address Of All People From Your Organisation Who Wish To Attend. South Gloucestershire Council, Have Over The Last Year, Been Developing A New Commissioning Model For Care And Support Services Delivered To People In Their Own Homes, Including Extra Care Housing Settings. The Model Aims To Promote Independence For People In Receipt Of Care To Enable Them To Live A Fulfilled Life, In Their Own Home, For As Long As Possible. The New Model Will Allow For Partnership Working Between Organisations In Different Parts Of The Sector Such As Traditional Home Care, Voluntary Care Sector Etc, To Deliver The Best Outcomes For Our Residents. The New Model Consists Of Three Parts; Home (care In A Person’s Home To Support With Regulated Care Needs), Community (connecting Our Residents To The Community And Organisations Within Their Community Who Can Support With Non-regulated Needs, Whilst Building Independence And Resilience), Partnership (forming Partnerships Between Organisations To Take A Strategic Approach To How Care & Support In A Persons Home Can Be Delivered To Ensure The Best Possible Outcomes For People). This Is An Exciting Opportunity To Change The Way In Which Care And Support In A Persons Home Is Delivered. The Opportunity Will Be Open For Tender Submissions, Later In September But We Would Like To Invite You Along To An Event, Where We Will Talk Through The Model In A Bit More Detail As Well As Explaining How The New Framework And Other Contracts, Called Off Of The Framework, Will Work.
