Background And Purpose In Reference To Csc-dole-doh Joint Memorandum Circular No. 1, S. 2020, Entitled “occupational Safety And Health Standards For Public Sector” And In Response To The Several Requests For A Health Break From Different Schools Due To The Increasing Number Of Cases Of Influenza - Like Illnesses Common During Rainy Season And Cold Weather, This Office, Through The Health And Nutrition Unit Recommends The Implementation Of The Following Preventive Health And Sanitation Activities In Schools And Offices In The Division Of Rizal. Ii. Qualifications Of The Service Provider 1. The Service Provider Should Have The Necessary Experience And Expertise In Providing Decontamination And Disinfection Services, Having Been Engaged In The Said Business For At Least Two (2) Years. 2. The Service Provider Must Be Philgeps Registered Or Must Have A Philgeps Registration Number. 3. The Service Provider Must Submit To The Silangan Elementary School The Hereunder Documentary Requirements: A. Certificate Of Satisfactory Performance Issued By Atleast Two (2) Current Or Previous Clients Attesting To The Services Rendered. B. Copy Of Company’s Current Certificate Of Product Registration Issued By The Doh – Bfad For The Chemicals To Be Used; And C. Relevant Trainings Attended During The Last Three (3) Years Of The Technicians Who Will Be Assigned To The Silangan Elementary School With Training Certificate/s As Proof. Iii. Duties And Responsibilities Of The Service Provider 1. Shall Ensure That The Service Provider Has Sufficient Technology And Manpower To Conduct Decontamination Activities That Would Eliminate And Possess High Bacterial Activity. 2. Shall Ensure That All Chemicals / Solutions And Materials Necessary For The Implementation Of Work Are Of The Best Quality, Safe For People And Environment, And Approved By The Bureau Of Food And Drugs (bfad) Or Certified By The Proper Government Authorities. 3. Shall Ensure That All Preparations And Formulations Are All Bio- Degradable, All Natural, And The Use Of The Solutions Are In Accordance With The Specifications Of Its Toxicologist And Entomologists. 4. Should Silangan Elementary School Find The Services To Be Ineffective As Evidenced By The Continuous Presence Of Crawling And Flying Insects, Silangan Elementary School Reserves The Right To Terminate The Contract With A Written Report Conveying The Grounds As To Why The Contract Needs To Be Terminated. The Service Provider Shall Respond Within Seven (7) Calendar Days From Receipt Of The Notice To Termination. If The Service Provider Fails To Show Cause After The Lapse Of The Seven (7) Day Period, Either By Inaction Or By Default, The Silangan Elementary School Shall Issue An Order Terminating The Contract; 5. Coordinate Its Work Schedule With The School Property Custodian, In Order Not To Disturb Or Disrupt The Working Schedule Of School Personnel. The Service Provider Shall Exercise Extraordinary Diligence In The Performance Of Its Services To Ensure That No Illness, Accident And/or Damage To Any Of Deped Employees Or Guests Will Take Place. The Service Provider Shall Assume Full Responsibility For Any Claim Or Liability That May Arise By Reason Of Illness, Accident And/or Damage Due To Any Or All Acts Of Omission, Negligence Or Fault Of The Service Provider And Its Agents, Thereby Rendering Silangan Elementary School Free And Exempt From Any Such Claim Or Liability. 6. Shall Provide/assign Service Technicians Who Are Honest, Reliable, Skilled, Well-trained And Certified Applicators/pesticide Handlers To Ensure Proper Application And Treatment. At Its Certified Applicators/pesticide Handlers To Ensure Proper Application And Treatment. At Its Own Expense, Shall Provide Its Personnel To Be Deployed In Silangan Elementary School, With Identification Card, Proper And Clean Uniform And Appropriate Personal Protective Equipment (ppe) And Clothing Wear While Performing Pest Control Services. Shall Submit A Weekly Service Decontamination And Disinfection Report To School Property Custodian Indicating Therein, The Areas That Were Treated And Monitored, Duly Confirmed/acknowledge By The School Representative Present During The Decontamination And Disinfection. Iv. Period Of Implementation The Contract Period Shall Be For Two (2) Month Commencing Upon The Date Stated In The Notice To Proceed (ntp). V. Scope Of Works The Service Provider Shall Provide All Labors And Materials, Tools And Equipment, Supervision And Other Incidentals For The Extermination And Effective Control Of All Disease-transmitting And Destructive Pests Such As Flies, Mosquitoes And Other Flying And Crawling Insects At Silangan Elementary School Compound. The Service Provider Shall Carry Out Their Pest Control Services In Accordance With The Pest Control Plan. 1.1 Disinfectant Misting/fogging 1.2 Termite Control 1.3 General Cleaning 1.4 Ultraviolet Light Sterilization/disinfection Vi. Area Of Treatment Coveraged (approximately 5000 Sq. M.) The Service Provider Shall Ensure That Treatment Covers The Following Areas / Premises Of Silangan Elementary School: A. All Areas/offices Inside The Silangan Elementary School, Including Canopies, Storage Areas And Roof Decks B. 61 Classrooms Within The 12 Buildings C. 60 Comfort Rooms D. Multipurpose Covered Court E. Silangan Elementary School Grounds Within The Perimeter Fence Vii. Chemicals To Be Used – Pesticides And Chemical Baits As Approved By The Fertilizer And Pesticide Authority (pfa) And Or Food And Drug Administration (fda). Submit Annex D – Chemicals To Be Used For Reference And Consideration. Viii. Terms Of Payment 1. Send Bill Arrangements 2. Payment Shall Be Made Based On The Actual Number Of Disinfection / Decontamination Conducted By The Service Provider And Upon Submission Of The Report, Stating That The Services Have Been Completed. Proposals, And To Annul The Procurement Process And Reject All Quotations / Proposal At Any Time Prior To Contract Award, Without Thereby Incurring Any Liability To The Affected Bidder Or Bidders. Deped Rizal – Silangan Elementary School Also Reserves The Right To Waive Any Required Formality In The Proposals Received, And Select The Proposal Which It Determines To Be The Most Advantageous To The Government.
