Valid Mayor’s Permit; B. Philgeps Registration Number; C. Omnibus Sworn Statement, Supported With Notarized Secretary’s Certificate Or Special Power Of Attorney **note: A Valid Certificate Of Platinum Membership With Philgeps May Be Submitted In Lieu Of Items A And B. Quotations Must Be Valid For A Period Of Ninety (90) Calendar Days From The Date Of Submission. Award Shall Be Made To The Lowest Quotation Which Complies With The Minimum Technical Specifications And Other Terms Stated Herein.
