Technical Specifications/scope Of Work And General Conditions Of The Project: Description / Scope Of Work Offered Technical Quotation/ Proposal Please Check The Box Corresponding To Your Offered Specifications I. Description Project Title: Procurement Of Preventive Maintenance Services Of One (1) Unit Elevator At Parkmall Satellite For Two (2) Years Number Of Unit/s: One (1) Unit Fuji Elevator Approved Budget Of Contract (abc): Php155,000.00 Ii. Scope Of Work The Contractor Undertakes, With Full Guarantee Against Unsatisfactory Workmanship, To Provide Regular Monthly Preventive Maintenance Services For A Period Of Two (2) Years And Ensure Compliance With The Following Responsibilities In Its Performance Of The Services: A. Preventive And Corrective Maintenance Services 1. Examine, Adjust And Lubricate All Appropriate Parts Of All Motors, Controllers, Brakes, Door Operators, Rail Guides And Lift Cars. 2. Inspect The Wear And Tear Condition Of The Equipment. 3. Clean And Adjust, As Necessary, All Machinery, Ropes, Sheaves Fixing, Controllers, Gates, Doors, Locks, Wirings, Motors And Safety Devices Of The Equipment. 4. Check All Machines And Components Of The Equipment For Abnormal Temperature Rise, Oil Leaks, Vibrations And Noise. 5. Provide The Necessary Grease Oil, Cotton Waste And Fuse Elements For Control Signals And Transformers, As Well As The Necessary Tools In Carrying Out The Said Work. 6. Conduct Inspection On The Following And Provide The Necessary Repairs/adjustments As Needed, To Wit: •indicator Lamps, Bulbs, Buzzers And Car Lights; •leveling Differences, Brake Slippage, Acceleration, Deceleration And Riding Comfort; •all Load Wires, Termination And The Operation Of Relays, Contacts, Push Buttons And All Safety Switches; 7. Clean The Machine Room, Hoist Way, Car Top (must Be Dry), Car Pit And Guide Rails. 8. Conduct A Once-a-month Examination And Testing Of All Safety Devices And Governors During Regular Visits. 9. The Contractor Shall Advise Pagcor On The Following: •any Defective And Worn-out Parts That Needs To Be Replaced. •the Cost Of The New Replacements And Details Of The Repair Work To Be Done. •the Specified Down Time To Be Undertaken Until The Equipment Is Brought To Its Normal Running Condition. 10. The Contractor Shall Supply And Install Genuine Elevator Spare Parts Indicated In The Service Report, With Reasonable And Acceptable Price And Lower Labor Cost Subject To The Approval Of Pagcor. Should Pagcor Opt To Purchase Replacement Parts From Other Suppliers, Such May Be Installed By The Contractor At Pagcor’s Cost. B. Emergency And Breakdown Repair Service The Contractor Shall Provide Emergency Support Services On An As-needed Basis. The Emergency Service Shall Be Available On A 24-hours Basis, Weekends, And Including Legal Holidays. The Contractor Shall Provide, Upon Immediate Notification From Pagcor, An Emergency Minor Adjustment Callback Service In Case Of Breakdown, Disorderly Operation, Or Malfunctioning Of The Equipment, Without Extra Charge To Pagcor. Contractor Shall Immediately Send Competent Technician/s On The Site For Emergency Action And On-the-spot Repair. All Work To Be Performed By The Contractor Shall Be Performed During The Contractor’s Regular Working Days And In Compliance With Pagcor’s Regular Operating Hours To Avoid Disruption Of Pagcor Operations. For Extra Services Requested By Pagcor Beyond The Regular Maintenance Service Covered By This Contract And Beyond The Contractor’s Regular Working Days And Hours, Overtime Rates Incurred By The Contractor’s Assigned Personnel In The Performance Of The Said Service Shall Be Charged To Pagcor. However, Payment Of The Overtime Rates Charged To Pagcor Shall Be Done Only Upon Verification And Confirmation Of The Services Performed. Overtime Rates To Be Charged By The Contractor Shall Be In Accordance With The Prevailing Rates. C. Support Service Requirements The Contractor Shall Establish A Single/centralized Point Of Contact Operation Center Or Help Desk Service With Hotline Numbers To Provide Timely And Responsive Trouble Reporting And Field Support For All Maintenance Related Issues. For Emergency Support Services Shall Be Phone-call Support And/or On-site And Available On A 24-hours Basis, Weekends, And Including Legal Holidays. The Contractor Shall Provide An Updated List Of Contact Persons, Telephones, And Cellphone Numbers In Case Of Emergency Calls. Repair And Response Time. The Contractor Shall Consider All Interruptions In Service As An Urgent Priority. Expected Response And Repair Time Are Given Below. Two (2) Hours Maximum For The Entrapped Passenger Twenty-four (24) Hours Maximum For The Equipment Breakdown If At Any Time During The Performance Of This Contract, The Service Provider Should Encounter Conditions Impeding Timely Performances Of Services, The Service Provider Shall Promptly Notify Pagcor In Writing Of The Fact Of The Delay, Its Cause And Its Likely Duration. Upon Receipt Of The Service Provider’s Notice, Pagcor Shall Evaluate The Situation And May Extend The Service Provider’s Time For Performance. Iii. Contractor’s Responsibilities The Contractor Represents And Warrants That It Has The Capacity To Perform Its Obligations And Undertakings According To The Terms And Conditions Of This Project, And Hereby Agrees And Warrants That It Shall Faithfully Observe And Comply Therewith. The Contractor Shall Provide Experienced And Qualified Manpower, Tools, Materials And Equipment Which Are Capable Of Producing The Quality And Quantity Of Work Required To Complete The Project As Specified In The Approved Scope Of Work And Specifications, Within Time As Specified In The Approved Contract. The Contractor Must Also Provide And Ensure That All Personnel Is Wearing Their Prescribed Uniform, Id And Proper Ppe At All Times And Use A Safety Device In Their Working Area To Avoid Any Accident. Provide Recommendations For The Schedule Of Preventive Maintenance Activity. Provide An Estimate/quotation Of The Cost Of Labor, Genuine Parts, And Materials For All Repair Services Within Two (2) Days After The Check-up. Minor Repairs And Adjustments As Required In The Periodic Schedule Shall Be Undertaken At No Extra Cost To The End-user. Checking And Servicing Of The Units Shall Be Done Under The Supervision Of The End-user. The Contractor Shall Be Responsible For All Injury To Persons And Damage To Pagcor And Other Property Caused By The Execution Of This Project. The Contractor Shall At All Times Be Directly Responsible For The Acts Or Conduct Of The Personnel Under Its Employ, For Their Salaries, Wages Or Compensation Or For Other Benefits Provided For Under Existing And Applicable Labor Laws. The Contractor Shall Strictly Observe The Sanitation, Safety And Security Rules And Regulations Of Pagcor During The Duration Of This Project. Iv. Submittals • Provide In Advance The Names Of Personnel That Will Conduct And Perform The Preventive Maintenance Services For The Processing Of The Necessary Work Permits. • Submit To The End-user The Duly Accomplished Unit Repair Service Report In Case Of Repair Services And Copies For Acknowledgment. Detailed Incident Report May Also Be Requested For Further Information Needed. • Site Inspection Certificate Issued By The End-user To Be Submitted During Submission And Opening Of Bids. V. Qualifications Of The Contractor • The Contractor Must Be Listed As Authorized Distributor, Competent And Must Have Been Engaged In The Business For The Last Five (5) Years And Performed Similar Services. • Two (2) Years Preferred Minimum Years For Completed Contracts Or Any Related Contracts To Be Provided By The Service Provider. • The Contractor Must Have At Least One (1) Authorized Service Center/office In Metro Cebu. Vi. Additional Terms And Condition Warranty: Warranty For All Parts And Services Shall Be Three (3) Months From The Acceptance Of The Items/services. Defective Items: Correction Of Defects Shall Be Within Seven (7) Calendar Days From The Receipt Of Notification. Inspection/test: Inspection And Testing For Materials And Workmanship Shall Be Conducted By The Sr. Facilities Management Officer (sfmo) And/or Any Authorized Representatives Of The Facilities Management Section To Confirm Their Conformity To The Technical Specifications. Payment Schedule: Pagcor Shall Pay The Contractor Every After Services Rendered. Delivery Schedule: Regular Monthly Services Within The Period Of Two (2) Years.
