Complete Blood Count (cbc) 2. Stool Examination (s/e) 3. Fasting Blood Sugar (fbs) 4. Lipid Profile (cholesterol, Hdl, Ldl, Vldl, Triglyceride) 5. 2-panel Drug Test (methamphetamine "shabu" & Tetrahydrocannabinol "marijuana" 6. Medical Consultation/ Complete Physical Examination 7. Urinalysis (u/a) 8. Chest X-ray 9. Visual Acuity For 35 Years Old And Above, Or If Prescribed 10. Electrocardiogram 11. Pap Smear (female Only) Terms And Conditions: 1. All Diagnostic Procedures Shall Be Done Within The Premises Of Peza Admin Bldg., Aai Compound, Gate 3, Mez, Brgy. Ibo, Lapu-lapu City, Cebu, In The Condition That The Service Provider Shall Provide Medical/examination Rooms/area For The Chest X-ray, Electrocardiogram And Pap Smear. 2. The Service Provide Shall Have The Following Attributes: A. Valid License To Operate Issued By The Department Of Health (doh) B. Has Provided Ape Services To At Least Ten (10) Companies Classified As Large Enterprises (employing At Least 200 Personnel) In The Last Five (5) Years. C. Fully Functional Equipment (i.e. X-ray, Ecg Machine, Blood Chemisty Analyzer) D. Courteous And Well-trained Personnel Composed Of At Least Three (3) Blood Extractors, At Least One (1) Medical Physician, One (1) Radio-technologist, And One (1) Manager To Oversee Their Staff. E. The Ape Service Will Be Available For Two (2) Consecutive Weekdays (december 05 & 06, 2024) At Peza Admin Bldg. - Mez, For The Diagnostic Procedure From 7:00 Am To 5:00 Pm. F. One-on-one Medical Consultation At Peza Admin Bldg., Mez For One (1) Day With At Least One (1) Medical Doctor On The Day Of The Release Of The Official Results 3. A Consolidated Report Of The Ape Providing The Physician's Findings, Patient's History And Laboratory Results Must Be Submitted By The Service Provider In Soft And Printed Copy To Mez-ehsd. The Service Provider Must Utilize The Computerized Systems For Client Database Build-up 4. The Service Provider Shall Have Measures In Place In Compliance With The Data Privacy Act To Protect The Rights Of Peza Employees This Shall Include: A. Securing Of Written Consent To Collect Personal Information Relevant To The Ape B. Safeguarding The Privacy And Security Of Personal Data Collected 5. The Service Provider Is Required To Submit The Following To Peza -mez Ehsd Within Ten (10) Working Days From The Date Of Ape: A. Schedule Of Proces Per Procedure Or Examination Conducted. B. Recommendations From The Examining/ Evaluating Physicians For Employees With Abnormal Findings In Their Ape C.hard Copies Of The Ape Result And The Consolidated Report Of Results And Findings D. Written Report Of Employees Who Failed To Undergo And/or Complete The Ape Terms Of Payment 1. Payment Shall Be Based On The Actual Cost Of The Specific Procedures Availed Of By The Employee 2. The Service Provider Shall Forward The Statement Of Account (soa) And Other Pertinent Documents Especially The Consolidated Report For The Services Rendered Within Ten (10) Working Days From The Last Scheduled Testing/screening Date. 3. The Ehsd Shall Review And Validate The Above-mentioned Documents Within Three (3) Working Days After The Receipt Of The Said Documents For Queries And/or Clarifications. In Case Of Errors (e.g. Double Billing, Erroneous Entries, Etc.) The Documents Shall Be Endorsed And Returned To The Service Provider For Appropriate Action 4. The End-user Shall Have A Period Of Fifteen (15) Working Days To Process Payment That Shall Commence After Completion Of The Review And Validation Of The Following Documents Submitted By The Service Provider: A. Statement Of Account B. Transmittal List That Shall Contain The Following Information - Name Of Employee, Date Of Examination/procedures, Types Of Examination And Medical Summary (class, Remarks And Attending Physician) 5.
