One (1) Lot Supply, Delivery, And Replacement Of One (1) Piece Inlet Guide Vane (igv) Actuator, Two (2) Pieces Input/output Board (iob), And One (1) Piece Pic5 Controller For Carrier Chiller Located At The Machine Room General Conditions: *to Ensure Attainment Of The Project Objectives, The Contractor Must Be The Authorized Service Provider Of The Brand Of The Equipment. A. Scope Of Work The Contractor Shall Supply All The Labor And Materials Necessary For The Satisfactory Completion Of The Project As Stated In The Scope Of Work. Parts Specifications/data: *the Contractor May Visit The Site To Inspect And Verify. *part/s Must Be Authentic Carrier Part/s. The Contractor Must Provide Authenticity Certificate Or Similar Documents. For Chiller Model: 19xrv525238huft64 1. Mobilization. 2. Preparation Of Tools And Equipment. 3. Switch Off Unit And Apply Lock-out Tag-out (loto). 4. Check Unit Operational Condition 5. Replacement Of Defective Parts; 5.1. Remove One (1) Piece Defective Igv Actuator. 5.2. Install One (1) Brand New Igv Actuator. 5.3. Disconnect Wiring Connection On Two (2) Pieces Iob. 5.4. Remove Two (2) Pieces Defective Iob. 5.5 Install Two (2) Pieces Brand New Iob. 5.6. Disconnect Wiring Connection On One (1) Piece Defective Display Controller (pic5). 5.7. Remove One (1) Piece Defective Display Controller (pic5). 5.8. Install One (1) Piece Brand New Display Controller (pic5). 6. Re-check Wiring Connections. 7. Check Configurations. 8. Start The Unit And Monitor. 9. Record Operating Parameters. 10. Submit Service Reports To The End-user. 11. Turnover Of Waste Materials To The End-user. 12. Turnover Of The Chiller To The End-user. 13. Demobilization. B. Project Schedule Project Duration Shall Be Thirty Five (35) Calendar Days Starting From The Issuance Of Contract And Presidential Security Command (psc) Security Clearance. Upon Acceptance Of Contract, As Prerequisite, The Contractor Shall Submit The Documentary Requirements (i.e. Letter Of Request For Access, Company Ids) For Processing Of Workers' Psc Security Clearance Through The Engineering Office. Contract Extensions Shall Be Upon A Written Request And Upon Approval Of The Head Of The Procuring Entity Or His/her Duly Authorized Representative After Consideration Of Reasonable And Justifiable Causes. C. Security Clearance The Contractor Upon Acceptance Of Contract, As Prerequisite, Shall Submit Within Five (5) Working Days Through The Engineering Office Together With The Documentary Requirements The Complete List Of Personnel/workers For The Processing And Issuance Of The Psc Security Clearance, To Wit: A. Letter Of Request For Access Addressed To The Head Of The Engineering Office (dir. Benefredo O. Bulatao) Indicating The Date And Time Of Service And List Of Personnel And Vehicle; B. Company Ids Of The Listed Personnel (clear Photocopy Both Sides); C. Vaccination Ids Of The Listed Personnel (clear Photocopy Both Sides); D. Or/cr Of Service Vehicle/s (clear Photocopies); And E. Driver’s License Of The Driver/s Of The Service Vehicle/s (clear Photocopies) It Is Understood That The Contractor Shall Strictly Comply Within The Prescribed Period The Submission Of The Said Requirements And Non-compliance Thereof, Is Not A Ground For A Contract Extension. D. Manpower Requirements The Contractor Shall Provide Op With Personnel Who Are Well-trained, Technically Skilled And Experienced In The Field Of Services Of Elevator. They Must Have A Supervising Engineer Or A Foreman And Skilled Installers/workers. All Workers/installers Must Be Provided With Their Official Uniform With Proper Identification And Ppe's While And During The Project Undertaking. In No Case Shall They Be Allowed To Stay Within Op Premises After Or Beyond Their Working Hours Unless Necessary And In Special Cases With The Approval Of The Appropriate Op Authorities And While In Their Respective Areas Of Work. E. Warranty The Contractor Shall Issue A Certificate Providing The Three (3) Months Warranty On Service/parts Supplied In This Project Against Factory Defects And Faulty Workmanship From Date Of Acceptance. The Warranty Shall Cover Services, All Replacement Parts.
