Description Provision Of On-site Annual Physical Exam For Ocd Co Permanent Employee On-site Ape (200 Pax) Service Provider Must: • Provide The Medical Supplies And Equipment Needed To Conduct The Test - Mobile Xray Machine - At Least 2 Ecg Machine • Provide Well-trained Medical Professionals For The Conduct Of Test • Cover All Other Expenses Associated With The Conduct Of Test • Ape To Consist Of Physical Examination, Chest Xray, Complete Blood Count, Blood Uric Acid, Creatinine, Urinalysis And Ecg • Over-all Physical Examination Report And Interpretation Of Results Must Be Certified By A Physician.
