This Is A Cardiovascular Disease (cvd), Chronic Kidney Disease (ckd) Hypertension And Diabetes Self-management Support Service. The Provision Of Essential Support Services Aimed At Empowering Individuals Living With These Chronic Conditions To Actively Manage Their Health And Well-being. The Service Will Focus On Increasing The Knowledge, Skills, And Confidence Of Its Users And To Promote Independence. The Provider Will Actively Mitigate Against The Participants Developing A Dependency On The Service By Promoting Independence And Exploring Ways For People To Implement And Sustain Activity Following Programme End.
