To Be Eligible To Participate, Interested Parties Are Required To Submit The Following Documents As Prescribed Under R.a. 9184, Otherwise Known As The Government Procurement Act, Viz : A. Department Of Trade And Industry (dti) Business Name Registration Or Sec Registration Certificate Whichever Maybe Appropriate Under Existing Laws Of The Philippines; B. Valid And Current Mayor's Permit/municipal License; C. Taxpayer's Identification Number Suppliers Already Accredited Need Not To Submit The Above Requirements. Bid Form Payment In The Amount Of 500.00 Will Be Collected Per Bac Resolution No. 25, Series Of 2011 For The Sale And Prescribing Of Prices Of Bidding Documents And Other Fees.
