Certificate Of Site Inspection Provided By The Municipality Of Pandi. 4. Certificate Of Brand-new Supply From The Manufacturer. 5. Certificate Of Spare Parts Availability For The Next Five (5) Years From The Manufacturer. 6. Certificate Of Authorization/dealership/distributorship From The Manufacturer. 7. Certificate Of Original Equipment Manufacturer. 8. Warranty Certificate From The Manufacturer. 9. Certificate Of Proof Of Establishment From The Manufacturer. 10. Proof That The Brand Of Led Wall Already Exists Here In The Philippines For More Than Five (5) Years. 11. Certificate Of Completion Of Training Issued By The Manufacturer Certifies That The Assigned Personnel Completed Training On The Installation/implementation Of Led Wall. 12. Sworn Affidavit/statement For The Employment Of The Personnel And Training Officer Assigned To The Project. 13. Sworn Affidavit/statement That The Company Has Been In The Led Business Industry For At Least Or More Than Five (5) Years. 14. The Contractor Shall Submit The Training Certificate Of The Assigned Authorize Managing Officer Or Safety Officer. 15. The Contractor Must Have A Licensed Engineer Specifically Ece 16. Certificate Of Dole No Pending Case/dole Nlrc (must Be Updated) 17. The Contractor Must Have At Least Three (5) Completed Installations Similar To The Project To Be Bid With The Offered Brand Of Led Wall Separate From The Slcc. 18.
