The Local Government Unit Of Gainza, Through Its Bids And Awards Committee (bac), Will Undertake The Purchase/service Of “office Equipment” In Accordance With Section 53.9, Small Value Procurement, Of The Revised Implementing Rules And Regulations (irr) Of Republic Act (r.a.) No. 9184 And Gppb Circular 09-2009. The Details Of The Project Are The Following: Name Of Project Purchase Of Office Equipment Delivery Site Lgu – Gainza, Camarines Sur Brief Description The Municipal Health Office Of Lgu–gainza Is In Need Of: Item/particulars Quantity 2hp Split Type Aircon 1 Delivery Period Within Ten (10) Calendar Days Upon Issuance Of Purchase Order Approved Budget Php52,498.00 Funding Source Mooe 2. The Duly Accomplished Price Quotation Form (annex “b”) Shall Be Submitted In A Sealed Envelope To The Bac On Or Before March 19, 2024, At Bac Office, 1st Floor, Municipal Building, Gainza, Camarines Sur. 3. All Quotations Shall Be Inclusive Of The Applicable Value Added Tax (vat) And Shall Be Valid For Ninety (90) Calendar Days From The Deadline Of Submission Of Quotations. 4. Any Interlineations, Erasures, Or Overwriting Shall Be Valid Only If They Are Signed Or Initialed By The Contractor Or His/her Duly Authorized Representative/s. 5. Liquidated Damages Equivalent To One-tenth Of One Percent (0.001%) Of The Value Of The Purchase Order Not Completed Within The Prescribed Completion Period Shall Be Imposed Per Day To Day Of Delay. The Lgu–gainza May Rescind The Agreement Once The Cumulative Amount Of Liquidated Damages Reaches Ten (10%) Of The Amount Of Purchase Order, Without Prejudice To Other Courses Of Action And Remedies Open To It. 6. The Project Shall Be Awarded To The Proponent Determined To Have Submitted The Complete And Lowest Quotation. The Winning Proponent Shall Furnish The Bac A Certified True Copy Of Its Valid And Current Documents Before The Award Of The Project, To Wit: A. Business Name Registration B. Business Permit C. Certificate Of Philgeps Registration 7. Lgu – Gainza Reserves The Right To Reject Any Of All Quotations At Any Time Prior To Award Of The Project Without Thereby Incurring Any Liability To The Affected Proponents And To Waive Any Minor Defects Therein, And To Accept Quotations As May Be Considered Advantageous To The Government.
