Introduction The Intent Of This Document Is To Outline The Requirements Of The Project And Secure Quotations From Surveyors To Undertake The Survey Services For The Relocation And Processing Of Land Title Of Various Lots Of Mapandan Water District, In Accordance With Republic Act (r.a.) No. 9184 (the Government Procurement Reform Act) And Its Implementing Rules And Regulations. 2. Property The Details Of The Properties Are The Following: Lot Area Acquired Address 600 Sq. M. Poblacion, Mapandan 115.7 Sq. M. Torres, Mapandan 3. Project 3.1 The Project Covers The Survey Services For The Land Segregation Survey Of The Above-mentioned Lot With A Total Area Of Approximately 715.70 Sq. Meters. 3.2 Respective Lot Monuments Of The Said Lots. Note: Interested Bidders May Conduct An Ocular Inspection Of The Project To Determine The Actual Site Condition And Verification Of Location Of Individual Lots. 3.3 Approval Of Individual Lots 3.4 Titling Of Individual Lots 4. Approved Budget For The Contract (abc) The Procuring Entity’s Approved Budget For The Contract (abc) Is Two Hundred Thousand Pesos (php200,000.00). The Abc Is Inclusive Of Value Added Tax, Withholding Tax, And Other Forms Of Taxes That May Be Imposed By The Philippine Government And/or Government Agencies In Relation The Above Project. 5. Scope Of Work 5.1 Land Record Research And Ocular Inspection Of The Area 5.2 Land Survey/ Segregation And Subdivision Survey 5.3 Preparation Of Plans And Preliminary Plotting Of Data Gathered And Installation Of Cylindrical Concrete Lot Monument Of Individual Lots. 5.4 Preparation Of Documents For The Issuance Of Survey Authority And Investigation Report To The Office Of The Cenr-officer, Dagupan City. 5.5 Submission Of Plans And Documents To Bureau Of Lands, San Fernando City, La Union For The Approval Of Subdivision Plan. 5.6 Individual Lot Titling. 6. Deliverables 6.1 Report Of The Relocation Survey Of Lots Listed In Items 5. 6.2 Individual Lot Plan Of The Subject Lot. 6.3 Installation Of Cylindrical Lot Monuments Of The Subject Lot. 6.4 Individual Approved Plan Of The Subject Lot. 6.5 Individual Certificate Of Land Title Of The Subject Lot. 7. Contract Of Duration 7.1 The Surveyor Shall Complete All Scope Of Work As Listed Under Items 5 Within The Period Of Three Hundred Sixty (360) To Seven Hundred Twenty (720) Calendar Days Or Two (2) Years. 8. Terms Of Payment 8.1 The Payment Term Is As Follows: 8.1.1 A Payment Equivalent To Thirty Percent (30%) Of The Contract Price Shall Be Released Upon Completion And Submission Of Items 5.1 To 5.4 Under Scope Of Work. 8.1.2 Additional Payment Equivalent To Thirty Percent (30%), Shall Be Released Upon Completion And Submission Of Approved Plan And Investigation Report Under Item 5.5 Of Scope Of Work. It Must Be Noted That If Partially Done, The Payment Must Be Pro-rated (per Lot Accomplishment). 8.1.3 Full Payment Equivalent To Forty Percent (40%) Shall Be Released Upon Completion And Submission Of Individual Certificate Of Land Title Or As Listed Under 5.6 Of Scope Of Work. It Must Be Noted That If Partially Done, The Payment Must Be Pro-rated (per Lot Accomplishment). 8.1.4 All Payments Shall Be Subject To Expanded Withholding Taxes And Other Application Taxes, And Such Other Deductions As May Be Mutually Agreed Upon By Both Parties. 9. Evaluation Guidelines Evaluation Guidelines Shall Be On The Basis Of The Lowest Calculated Responsive Bid Pursuant To Section 32 Under Rule Ix, Of The Implementing Rules And Regulations Of Ra 9184. Moreover, Price Escalation Is Not Allowed As Per Section 61.6 Of Irr States That “for The Given Scope Of Work In The Contract As Awarded, All Bid Process Shall Be Considered As Fixed Prices, And Therefore Not Subject To Price Adjustment And Escalation During Contract Implementation Except Under Extraordinary Circumstances And Upon Prior Approval Of The Gppb, Or When A Treaty Or International Or Executive Agreement Expressly Allows It Pursuant To Section 4 Of This Irr”.
