Moreover, Eligibility And Rfq Documents To Be Submitted Are: (i) Photocopy Of Proponent’s Dti Or Sec Registration Certificate, As The Case Maybe; (ii) Current And Valid Mayor’s Permit; (iii) Tax Clearance As Per Executive Order 398, Series Of 2005 As Finally Reviewed And Approved By The Bir (iv) Certified Copy Of Bidder’s Latest Audited Financial Statement, Income/business Tax Returns (v) Philgeps Certificate Of Registration And Membership In Accordance With Section 8.5.2 Of The Rirr (vi) Notarized Omnibus Sworn Statement In Accordance With Section 25.3 Of The Rirr (vii) Completely Filed-up Request For Quotation In The Prescribed Form 5.
