Description Technical Specifications Cy 2024 Annual Medical And Physical Examination (ampe) For Branch Office Employees I. Introduction The Annual Medical And Physical Examination (ampe) Is A Mandatory Tool To Monitor The Health Status Of Employees (job Grade Of 16 And Below) By Providing Diagnostic Services Like Routine Diagnostic, Screening And Other Ancillary Procedures To Ensure Early Detection And Adequate Treatment Of Any Illnesses. The Ampe Is Included Under The Gsis Health And Wellness Program As Contained In The Policies And Procedural Guidelines No. 246-13 Approved By The Gsis Board Of Trustees Under Resolution No. 112 Dated 10 October 2013. The Conduct Of The Ampe Shall Commence On The Month Of June-july Within The Current Calendar Year While The Random Drug Test Shall Be Conducted All Throughout The Year. Employees May Opt To Undergo The Ampe On Weekdays Or On Weekends At Designated Accredited Clinics. Ii. Coverage A Gsis Employee With Jg 16 And Below, Assigned In Gsis General Santos Branch Office Holding A Permanent, Temporary Or Co-terminus Appointment, And Who Is In The Service As Of The Date Of Effectivity Of This Agreement Shall Be Covered. Exempted Are The Newly-hired Employees Who Already Underwent The Required Pre-employment Medical Screening And Examinations During The Current Year. They Shall Be Required To Undergo The Ampe On The Succeeding Year From The Time They Are Appointed But Will Be Required To Undergo The Random And Mandatory Drug Screening Test. Iii. Scope Of Work A. The Following Required Diagnostic And Screening Procedures/examinations Shall Vary Depending On The Specific Age Bracket; 1. Bracket 1: Age 30 Years Of Age And Below (male And Female); A. Physical Examination B. Complete Blood Count With Platelet Count C. Blood Chemistry (fasting Blood Sugar And Total Cholesterol): D. Routine Urinalysis E. Chest X-ray (plain); Posterior-anterior (pa) View F. 12-lead Electrocardiogram (ecg) G. Dental Examination H. Basic Optical Examination (visual Acuity View) 2. Bracket 2: Age 31 To 45 Years Of Age (male And Female); A. Physical Examination B. Complete Blood Count With Platelet Count C. Blood Chemistry (fasting Blood Sugar, Lipid Profile, Total Cholesterol And Uric Acid Determination) D. Routine Urinalysis E. Chest X-ray (plain); Posterior-anterior (pa) View F. 12 Lead Electrocardiogram (ecg) G. Dental Examination H. Basic Optical Examination (visual Acuity View) 3. Bracket 3: Age 46 Years Of Age And Above (male And Female); A. All Procedures And Examinations Mentioned Above B. Blood Chemistry To Include: Liver Enzymes (sgpt And Scot) And Lipid Profile (triglycerides, Hdl, Ldl, Vldl), Blood Urea Nitrogen And Creatinine C. Hba1c D. Serum Electrolytes (na+, K+ And Ca+) E. Breast Ultrasound For Female, 46 Years Old And Above F. Prostate Specific Antigen (psa) For Male, 50 Years Old And Above B. The Following Diagnostic Procedures/laboratory Tests Shall Be Included As Follow-up Examinations After Abnormal Findings: Abnormal Results - Follow-up Tests 1. Suspicious Densities In Chest X-ray - Chest X-ray Apico-lordotic View 2. For Bracket 1 Employees Only Who Have Rendered An Impaired Fasting Glucose And Elevated Fasting Blood Sugar Results - Hba1c 3. Bracket 1 Who Are Overweight/obese And/or With Personal Medical History Of Hypercholesterolemia Or Dyslipidemia - Lipid Profile 4. Abnormal Urinalysis Results - Repeat Urinalysis Iv. Terms And Conditions A. The Accredited Diagnostic And Multi-specialty Clinics Must Have The Following Attributes: 1. Duly Accredited Or Licensed By The Department Of Health (doh) Both As A Diagnostic Laboratory And As A Drug Testing Clinic; 2. Fully-functional Equipment (i.e. X-ray And Ecg Machine, Blood Chemistry Analyzer); 3. Clean, Sanitized And Well-maintained Facilities (air-conditioned, Preferably With Two Separate Comfort Rooms For Male And Female Patients); 4. Courteous And Well-trained Staff But Not Limited To: One (1) Physician, Receptionist, Radio Technologist, Medical Technologist, Optometrist, And Two (2) Nurses; And Should Be Covid-19free 5. Available And Open To Employees From 7:30am Up To 7:30pm (monday To Sunday). B. The Schedule Of The Ampe Shall Be Arranged By The Branch Head With Concurrence Of The Concerned Vice President And/or Senior Vice President. The Branch Head Shall Endorse To The Service Provider Coordinator The List Of Employees Who Shall Undergo The Ampe And To The Scheduled Date. C. A Consolidated Report (annex 2) Of The Ampe Detailing The Physician’s Findings, Patient’s History And Laboratory Results Must Be Submitted By The Service Provider In Soft And Printed Copy Directly To The Branch Head Before Payment Be Done. The Bo Head Shall Forward The Consolidated Report To The Medical And Wellness Services Department (mwsd) Not Later Than 31 December 2024 Of The Current Year. All Medical Records, Laboratory Results And Other Important Information Obtained By The Service Provider Shall Be Treated With Confidentiality. D. The Service Provider Shall Not Use Or Disclose Such Confidential Information, Or Any Part Thereof, In Any Manner Other Than Is Necessary To Perform Its Services Under This Agreement Or As Required By Law. Relative Thereto, The Gsis Agrees To Support And Coordinate With The Service Provider, Its Directors And/or Officers, Employees, Or Duly Authorized Representatives, In Their Defense Against Any Action, Sum Of Money, Liability, Damages, And Claims Which Any Third Party May Bring Against The Service Provider As A Direct Or Indirect Result Of The Gsis Having Received And/or Used The Confidential Information. E. The Service Provider Shall Secure The Written Waiver Of Rights From The Employees Of The Right To The Confidentiality Of Information Between The Physician And Patient For Procedures And Examinations Undertaken In Behalf Of The Gsis. The Written Waiver Shall Include The Authorization For The Service Provider To Submit To The Gsis All Documents For Incorporation In The Health Profile Of The Employees. F. All Documents, Records, Reports, Receipts And Information About The Ampe Including Those Recorded In Database Systems Of The Service Provider Shall Be The Property Of The Gsis. G. The Service Provider Is Required To Submit The Following To The Gsis Bo In Accordance With The Prescribed Period: 1. Schedule Of Prices Per Procedure/examination: 2. Submission Of The Following Documents/requirements To The Branch Manager, General Santos Branch Office: A. Ampe Results Within Two (2) Weeks Or 14 Calendar Days (cds) From The Time Of Conduct Of The Ampe. The Sp Must Ensure Prompt Submission Of The Results To Be Placed In Sealed Envelopes. B. Conduct Follow-up Laboratory Tests (with Abnormal Findings) Within Seven (7) Cds After The Initial Result. Full Coordination To Branch Office Mus C. Results Of Follow-up Laboratory Tests Within Seven (7) Cds After The Conduct Of Laboratory Tests, D. One (1) Hard Copy Of The Individual Ampe Result Delivered After Two (2) Weeks/14 Cds From The Time Of The Conduct Of The Ampe Including The Soft Copies In Pdf Format. (one Copy For The Employee And Soft Copy Emailed To Gsis Infirmary For Record Profiling). E. Executive Summary / Post Implementation Report, Statistical Report Indicating The Trend Of Illnesses (hpn, Dm, Dyslipidemia) And Lifestyle Disease (obesity, Underweight) Found Among The Gsis Employees And Their Recommendation, Submitted Three (3) Weeks After The Final Conduct Of The Ampe. F. Consolidated Report In Excel Format (annex A) Indicating The Laboratory Values/results Of The Tests Conducted Detailing The Physician’s Findings, Including Any Disabilities, Patient’s History And Recommendation In Soft And Printed Copy Submitted After Three (3) Weeks From The Final Conduct Of The Ampe. G. Weekly Email And Written Report Of Employees Who Failed To Undergo And/or Complete The Ampe At The End Of Each Scheduled Week. H. Non-compliance With The Aforementioned Provisions Is Subject To Penalties As Stated Under Item Vii. Penalty Clauses V. Terms Of Payment 1. The Payment Shall Be Based On The Actual Number Of Employees Who Underwent Ampe Procedures And Shall Only Be Processed After The All The Necessary Documents And Requirements Have Been Provided. The Service Provider Shall Forward The Statement Of Account For The Services Rendered Within Two (2) Weeks From The Last Scheduled Testing/screening Date. 2. The Gsis Shall Review And Validate The Above-mentioned Documents Within Thirty (30) Working Days After The Receipt Of The Said Documents For Queries And/or Clarifications. In Case Of Errors (i.e. Double Billing, Erroneous Entries, Etc.) The Documents Shall Be Endorsed And Returned To The Service Provider For Appropriate Action. 3. The Gsis Shall Have A Period Of Thirty (30) \working Days To Process Payment That Shall Commence After Completion Of The Review And Validation Of The Following Documents Submitted By The Service Provider: A. Statement Of Account (soa); B. Procedure/examinations Done And C. Transmittal List That Shall Contain The Following Information: Name Of Employee, Date Of Examination/procedures, Types Of Procedures/ Examination And Payment Due For Each Employee. 4. All Payments Shall Be Subject To Existing Accounting And Auditing Rules Applicable To Gsis And Subject To Applicable Tax. 5. The Service Provider Agrees That Any Payment Due And Payable And/or May Be Due The Service Provider Shall Be Offset Against Any Amount Due To The Gsis Under The Agreement, Such As But Not Limited To Liquidated Damages. Vi. Contract Period A. The Duration Of The Project Shall Commence Upon The Issuance Of A Notice Of Award But Not To Exceed Two (2) Months But In No Case Shall Go Beyond 31 December 2024 And/or Completion Of Services And Obligations Of Both Parties. B. Notwithstanding Any Provisions To The Contrary, The Gsis Shall Have The Right, Power And Privilege To Terminate The Services Of The Service Provider For Any Justifiable Cause Whatsoever Without Need Of Judicial Action By Giving Thirty (30) Calendar Days Written Notice To That Effect To The Service Provider, Which Hereby Agrees To Abide By The Decision Of The Gsis Vii. Penalty Clauses A. When The Winning Bidder Fails To Satisfactorily Deliver Goods Under The Contract Within The Specified Delivery Schedule, Inclusive Of Duly Granted Time Extensions, If Any, The Supplier Shall Be Liable For Damages For The Delay And Shall Pay Gsis Liquidated Damages, Not By Way Of Penalty, An Amount Equal To One-tenth (1/10) Of One Percent (1%) Of The Cost Of The Delayed Goods Scheduled For Delivery For Every Day Of Delay Until Such Goods Are Finally Delivered And Accepted By Gsis. Gsis Need Not Prove That It Has Incurred Actual Damages To Be Entitled To Liquidated Damages. Such Amount Shall Be Deducted From Any Money Due Or Which May Become Due To The Supplier, Or Collected From Any Securities Or Warranties Posted By The Supplier, Whichever Is Convenient To The Procuring Entity Concerned. In No Case Shall The Total Sum Of Liquidated Damages Exceed Ten Percent (10%) Of The Total Contract Price, In Which Event The Procuring Entity Concerned May Rescind The Contract And Impose Appropriate Sanctions Over And Above The Liquidated Damages To Be Paid. B. In Case The Sp Fails Or Refuses, For Any Reason Whatsoever, To Render The Required Procedures As Scheduled, The Sp Shall Be Liable And Pay In Favor Of The Gsis The Full Amount Allowed For The Said Services And/or Other Expenses Incurred By Reason Of Such Failure. C. The Sp Shall Not Assign Or Sub-contract The Ampe Services Without The Written Approval Of The Gsis. Violation Of This Condition Shall Be A Ground For The Cancellation Of The Contract By The Gsis. D. Only Department Of Health Duly Licensed/accredited/authorized Personnel Of The Sp Are Allowed To Participate In The Conduct Of The Ampe In Gsis. Violation Of This Provision Shall Be A Basis For Cancellation Of The Contract And Payment Of Any Damages Incurred As A Result Thereof. E. The Gsis Shall Have The Right To Pre-terminate This Contract In Whole Or In Part For Default Of The Service Provider Or Breach Or Violation Of The Terms And Conditions Of This Contract Without Need Of Judicial Action Or For Just Cause To Be Determined By The Gsis, Which Determination Shall Be Final And Binding To The Service Provider. Viii. Data Privacy The Winning Bidder/supplier Must Comply With All The Provisions Of Republic Act No. 10173 Known As The “data Privacy Act Of 2012” And Its Implementing Rules And Regulations. Ix. Non-disclosure Agreement The Winning Bidder/supplier Shall Protect Confidential Information About The Gsis, And To Use It Only For Purposes Of Complying With Its Tasks And Obligations Under The Official Bid Document/contract/purchase Order. It Shall Not Disclose Any Confidential Information About The Gsis Project Or Procurement To A Third Party Without The Consent Of The Gsis. “confidential Information” Refers To Information Concerning The Affairs Of The Gsis Which Are Not Available To The Public Which, If Disclosed, May Compromise The Security Of The System. The Winning Bidder/supplier Shall Not Disclose Any Confidential Information Acquired During Its Engagement With Gsis Even After The Expiration Or Termination Of The Contract. X. Approved Budget The Approved Budget Is Php186,700.00
