Description Request For Quotation The Department Of Tourism-region 10, Through Its Bids And Awards Committee (bac), Invites All Interested Philgeps Registered Suppliers To Submit Their Lowest Price Quotation /bid Proposals On The Procurement Of The Project Mentioned Below Under Small Value Procurement (sec.53.9) Of Ra 9184 Subject To The Terms And Reference (tor): Name Of The Project Office Security Services Description Please Refer To The Terms Of Reference (tor) Delivery Date: January To December 2025 Terms Of Reference: 1) The Amount To Be Paid By The Procuring Entity For The Security Services Must Be On A Monthly Basis Upon Submission Of Statement Of Account (soa). 2) The Interested Supplier Must Meet The Following Requirements: A) Must Be A Member Of Philippine Association Of Detective Protective Agency Operators (padpao). B) Must Provide Two (2) Licensed Security Guards On A 12 Hour Shifting Schedule Per Guard. C) The Security Guards Must Have Passed The Neuro Psychological Test, With Good Moral Character, Physically And Mentally Fit, With Age Not Less Than 21 Years Old. D) Must Provide Basic Benefits Prescribed By Law (13th Month Pay, Sss, Phic, Hdmf). 2) The Interested Suppliers Are Required To Submit The Following Eligibility Documents: A) Copy Of Philgeps Registration B) Copy Of Updated Business / Mayor’s Permit C) Copy Of Dti Or Sec Registration D) Copy Of Bir Certificate Of Registration / Tax Clearance Certificate E) Copy Of Annual Income Tax Return (for Abc Above Php 500,000.00) F) Notarized Omnibus Sworn Statement G) Bid Proposal With Quotation H) Copy Of Updated Security Guard’s License I) Copy Of Latest Neuro Psychological Test Of The Security Guards. J) Copy Of Sss, Phic, Hdmf Compliance Certificate K) Copy Of Padpao Membership 3) The Bid Proposals Together With The Eligibility Documents Must Be Submitted To The Bac Secretariat, Department Of Tourism Regional Office 10, Rosario Crescent, Limketkai Center, Cagayan De Oro City. 4) Interested Suppliers May Obtain Further Details Of The Project From The Dot-10 Bids And Awards Committee During Office Hours 08:00am To 05:00pm, Mondays To Fridays Except Holidays.
