The Service Provider Shall Provide A Professional Unit Or Group Who Shall Conduct The Onsite Drug Tests At The Dilg Central Office Premises. They Shall See To It That Members Of The Team Are Free From Covid-19 And Any Contagious Diseases For The Duration Of The Onsite Drug Screening Test. 2. The Drug Test Schedule Shall Be Arranged And Supervised By The Dilg-co Clinic Personnel. The List Of Employees Who Shall Undergo The Drug Test On The Screening Day Shall Be Endorsed By The Dilg Clinic To The Service Provider. 3. The Service Provider Must Have A Certificate Of Accreditation For Drug Testing Laboratory Issued By The Department Of Health (doh) And Shall Follow The Existing Doh Guidelines And Procedures On Drug Screening Services. "4. The Service Provider Must Be Capable Of Performing Two (2) Drug Parameters: Tetrahydrocannabinol (thc) “marijuana” And Methamphetamine Hydro-chloride (shabu). " 5. The Service Provider Shall Provide The Following Supplies And Equipment Necessary For The Collection Of Urine Specimen To Wit: "a. Tissue Paper B. Isopropyl Alcohol 70% C. Plastic Cups And Trash Bags D. Five (5) To Ten (10) Laptops To Be Used For Encoding And Must Be Doh-registered; Each Laptop Is Assigned With Encoder/personnel Of The Service Provider. " 6. The Service Provider Must Serve And Screened 100% Of The Total Employees Of The Dilg-co, Regardless Of Their Appointment Status 7. The Service Provider Shall Not Accommodate Employees Who Are Not Endorsed/ Scheduled To Take The Drug Screening. 8. The Drug Test Screening Will Be Executed Within One (1) To Three (3) Working Days. Dilg-co Personnel Who Were Not Able To Take The Test During That Period Will Be Endorsed By The Dilg Clinic And Will Be Advised To Proceed To The Testing Center Of The Service Provider. The Service Provider Should Accommodate Dilg-co Personnel Endorsed By Dilg Clinic To Undertake The Test At Their Laboratory Facility. 9. The Service Provider Shall Submit The Drug Test Results To The Dilg-co Clinic In A Sealed Envelope, Within Five (5) To Ten (10) Calendar Days After The Administration Of Drug Test. 10. Payment For Confirmatory Test Shall Be Shouldered By The Service Provider, In Case Of Any Positive Drug Test Results. 11. All Drug Test Result And Any Other Important Information Obtained By The Service Provider, Including Personal Information Of Dilg-co Personnel Shall Be Treated By The Service Provider With Utmost Confidentiality.
