The Department Of Science And Technology, Caraga, Through Its Bac Intends To Procure Professional Services For The 17th Pnhrs Week Documentation Services. Duration Of Engagement: Maximum Of Three (3) Months A. Scope Of Work: The Duties And Responsibilities Of The Service Provider Include But Not Limited To: 1. Coordinate Closely With Pchrd And Chrdc Through Dost Caraga Throughout The Duration Of The Engagement (planning, Development, Implementation, And Finalization/ Completion Of The Project). 2. Prepare And Submit An Inception Report That Will Include The Detailed Plans And Timeline For The Implementation Of The Project; 3. Provide Regular Updates To Pchrd And Chrdc Through Dost Caraga On The Status Of The Project; And 4. Submit All Required Deliverables On The Agreed Timeline. B. Outputs And Deliverables: (a) Inception Report And (b) 17th Pnhrs Week Event Report (in Digital Lay-out) ***other Detailed Scope Of Work, Deliverables, Requirements Etc. Are Indicated In The Attached Terms Of Reference. The Service Provider Must Submit The Following Requirements: 1. Cvs Of Technical Personnel Proposed For This Project Highlighting Qualifications And Experience In Similar Projects; 2. Work References – Contact Details (email Addresses) Of Referees (firms For Whom The Service Provider Has Produced Similar Assignments); And 3. Samples Of Recent Similar Undertakings, Online Portfolios, And Links To Event Proceedings And Other Related Works. 4. Registration Certificate From Sec, Dti For A Sole Proprietorship, Or Cda For Cooperatives. 5. Valid Mayor’s/ Business Permit With Kind Of Business. 6. For Individual Consultants Not Registered Under A Sole Proprietorship, A Bir Certificate Of Registration Shall Be Submitted, In Lieu Of Dti Registration And Mayor’s / Business Permit. 7.
