Conduct Firm Visit, Collection Of Data And Submit Inception Report 2. Prepare And Submit Progress Report To Dost Caraga Clients 3. Conduct One-on-one Presentation And Discussion Of Progress Report 4. Conduct Face To Face Monitoring Of Firms Compliance To Consultant’s Recommendation. 5. Presentation On Final Output To Dost Caraga & Clients. D. Required Reports 1. Four (4) Printed And Two (2) Usb Copies Of The Inception Reports For Thirty-five (35) Msmes 2. Four (4) Printed And Two (2) Usb Copies Of The Progress Reports For Thirty-five (35) Msmes 3. Four (4) Printed And Two (2) Usb Copies Of The Validated Final Reports For Thirty-five (35) Msmes • The Consultant Shall Conduct A Face-to-face Consultancy Visit And Will Shoulder The Expenses Of Their Food, Air Fare And Hotel Accommodation. The Consultant Must Submit The Following Requirements: 1. Updated Professional License/ Curriculum Vitae With A Letter Of Intent (for Accreditation Of Consultant); 2. Performance Rating Of Very Satisfactory (vs) From Another Agency Which Focuses On Consultancy Services For Manufacturing Productivity Program; 3. Sample Of Final Report For Consultancy Services For Manufacturing Productivity Program; 4. Registration Certificate From Sec, Dti For Sole Proprietorship, Or Cda For Cooperatives; 5. Valid Mayor’s/ Business Permit With Kind Of Business (consultancy Services); 6. For Individual Consultants Not Registered Under A Sole Proprietorship, A Bir Certificate Of Registration Shall Be Submitted, In Lieu Of Dti Registration And Mayor’s / Business Permit; 7. Philgeps Registration Certificate; 8. Tax Clearance Per E.o. 398 S.2005; 9. Business/ Income Tax Return; And 10. Omnibus Sworn Statement (unnotarized) - To Submit Notarized Copy Upon Noa.
