Description A. Submission Of Requirements 1. Bidder Shall Attach To The Price Quotation Form (annex A) The Following Documentary Requirements: (a) Philgeps Certificate Of Registration/philgeps Certificate Number; (b) Mayor’s Permit/business Permit. (c) Omnibus Sworn Statement 2. The Price Quotation Form Shall Be Signed By The Owner/proprietor/president Or In Case Of Representatives, A Special Power Of Attorney Shall Be Attached. For The Authorized Representatives Of A Partnership, Corporation And/ Or Cooperatives, A Notarized Secretary’s Certificate Or Board Resolution Shall Be Attached. 3. For Electronic Submission- All The Required Documentary Requirements As Provided Under Section A.1 And The Duly Signed Price Quotation Form Shall Be Submitted In Pdf File And In The Following Format: Email Subject: Supply, Delivery, And Installation Of Four Air-conditioning Units 2.5hp Email Body: To The Bids & Awards Committee, Doh- Lpdatrc (1) Rfq Number Of The Project: (2) Name And Business Address Of The Supplier: (3) Email Address And Contact Number Of The Supplier: B. Proper Sealing And Marking Of Quotations- For Physical Submission Interested Bidders Are Required To Accomplish, Provide Correct And Accurate Information, And Submit One (1) Original And Additional Two (2) Certified True Copies Of Their Duly Signed Price Quotation Form (annex “a”) Using Their Official Letterhead And Other Documentary Requirements As Provided In Section A.1. And Enclose The Same In One (1) Sealed Envelope. The Sealed Envelope Shall Be Marked As To Include The Following: (1) Name And Address Of The Doh – Las Piñas Drug Abuse Treatment And Rehabilitation Center Bids And Awards Committee, (2) Title And Abc Of The Project, (3) Rfq Number Of The Project, And (4) Name And Business Address Of The Supplier. (5) Email Address And Contact Number Of The Supplier The Improper Sealing And/or Marking Of The Quotations Shall Not Disqualify A Prospective Bidder. However, The Bids And Awards Committee Shall Assume No Responsibility For The Misplacement Of The Contents Of The Improperly Sealed Or Marked Quotation. C. Amount And Denomination Of Quotation Price Quotations Exceeding The Abc Shall Be Rejected. All Price Quotations Shall Be Quoted In Philippine Peso. D. No Multiple Submission Of Quotations Multiple Submission Of Quotations Shall Not Be Allowed. If During The Opening Of Quotations, A Supplier Is Found To Have Submitted Two Or More Different Quotations For The Same Project, All Submitted Quotations Of The Prospective Supplier Shall Be Disregarded And Shall Be Returned To The Latter Unopened. E. Validity Of Quotation Price Quotations Must Be Valid For A Period Of Thirty (30) Calendar Days From The Date Of Submission Of Quotations. F. Alterations, Erasures And Overwriting Any Alterations, Erasures Or Overwriting Shall Be Valid Only If Signed Or Initialed By The Bidder Or His/her Duly Authorized Representative/s. G. Right Of Inspection The Doh-lpdatrc Shall Have The Right To Inspect And/or Test The Goods To Confirm Their Conformity With The Technical Specifications. H. Award Of Contract Award Of Contract Shall Be Made Per Lot Which Is Found To Be The Lowest, Responsive And Compliant With The Minimum Technical Specifications And Other Terms And Conditions Specified Herein. I. Warranty Security The Supplier Shall Be Required To Put Up A Warranty Security In The Form And Amount As Allowed Under Sec. 62.1 Of The Irr Of R.a. No. 9184, To Wit: For The Procurement Of Goods, In Order To Assure That Manufacturing Defects Shall Be Corrected By The Supplier, A Warranty Security Shall Be Required From The Contract Awardee For A Minimum Period Of Three (3) Months, In The Case Of Expendable Supplies, Or A Minimum Period Of One (1) Year, In The Case Of Non-expendable Supplies, After Acceptance By The Procuring Entity Of The Delivered Supplies. The Obligation For The Warranty Shall Be Covered By Either Retention Money In An Amount Equivalent To At Least One Percent (1%) Of Every Progress Payment, Or A Special Bank Guarantee Equivalent To At Least One Percent (1%) Of The Total Contract Price. The Said Amounts Shall Only Be Released After The Lapse Of The Warranty Period Or, In The Case Of Expendable Supplies, After Consumption Thereof: Provided, However, That The Supplies Delivered Are Free From Patent And Latent Defects And All The Conditions Imposed Under The Contract Have Been Fully Met. J. Liquidated Damages Liquidated Damages Equivalent To One Tenth Of One Percent Of The Value Of Goods Undelivered Within The Prescribed Delivery Period Shall Be Imposed For Every Day Of Delay. The Doh – Lp Datrc Shall Have The Right To Rescind The Contract When The Cumulative Amount Of Liquidated Damages Reaches Ten Percent (10%) Of The Contract Price, Without Prejudice To Other Courses Of Action And Remedies Available To It.
