Submission Of Requirements 1. Sealed Quotations And Other Requirements Stated Below Shall Be Submitted To Dar Tanza, Boac, Marinduque, Philippines Addressed To “the Chairman- Dar Provincial Office Bids And Awards Committee (bac). 2. Supplier Shall Submit The Following Requirements If The Amount Is More Than P50,000.00: A. Duly Signed Request For Quotation. Prices Shall Be Quoted In Philippine Peso. Statement Of Compliance Must Be Accomplished By Supplier With Either Complied Or None. B. Latest Philgeps Registration Certificate. C. Valid Mayor’s Permit, Tax Clearance Certificate/income Tax Return Filed And Paid Through Bir Electronic Filing Payment System (efps) D. Omnibus Sworn Statement B. Award The Supplier/service Provider Who Submitted The Lowest Calculated Responsive Quotation Shall Be Awarded Of The Contract After Evaluation By The Dar Provincial Office Bids And Awards Committee C. Delivery 1. Provision And Delivery Of The Services Rental Survey Equipment With Operator Shall Be Made Within 7 Calendar Days From Date Of Receipt Of Notice Of Award. 2. Survey Equipment Rental Service Shall Be Served In The Six (6) Municipalities Of Marinduque Hereto Defined As The Project Site, Cost To The Account Of Supplier. Survey Output Per Day Shall Not Be Less Than 10 Hectares. 3. The Service Provider/supplier Shall Deliver The Services To The Project Site And Shall Coordinate With The Survey Team Of Dar Marinduque And Split Parcelization Coordinator For Every Schedule Of Survey And Subject Landholdings Province Wide. 4. The Service Provider Shall Report To Dar Thru The Parcelization Coordinator The Daily Status And Accomplishment Of Lots Surveyed. D. Evaluation Of Quotations Quotations Shall Be Compared And Evaluated On The Basis Of The Following Criteria: 1. Completeness Of Submission 2. Compliance With Technical Specifications 3. Price 4. Add-on Services E. Instructions A. Supplier Shall Be Responsible For The Source(s) Of Its Equipment, And Shall Ensure Timely Delivery Of Survey Outputs Within The Contract Period. Failure Of The Supplier To Comply With This Provision Shall Be Ground For Cancellation Of The Award. B. Operator Shall Be Equipped With Technical Knowledge On How To Operate The Equipment/ Device. C. Operator Shall Be Responsible For The Safety And Operation Of Rented Equipment/s. D. Supplier/operator Shall Accomplish At Least 10 Hectares Per Day But Not Less Than 501 Hectares For 40 Days. E. All Duties, Excise, And Other Taxes, And Revenue Charges Shall Be Paid By The Supplier. F. As A Pre-condition To Payment, Completed Survey Report Duly Signed By The Parcelization Coordinator And Municipal Agrarian Reform Program Officer And Other Necessary Documents Shall Be Submitted By The Supplier To The Department Of Agrarian Reform. G. All Transactions Are Subject To Withholding Of Credible Value Added Tax And/or Expanded Value Added Tax Per Revenue Regulation(s) Of The Bureau Of Internal Revenue. H. All Costs Of The Necessary Rental Services On The Equipment/device Shall Be To The Account Of Suppliers. F. Payment One Hundred Percent (100%) Of The Contract Price Less Government Taxes Shall Be Paid To The Supplier Upon The Submission Of The Necessary Documents As Stated In E.6. G. Fund Source – Project Split (lp) – Current Year 2024 K.
