As Applicable For Non-recurring Charges.  Clin/slins Shall Include Item Descriptions, Quantities And Units Of Measure.  The Requested Service Date And Acceptance Criteria Are Outlined In The Attached Inquiry.  The Product And Service Code For This Telecommunications Requirement Is D304.
