The Supplier With The Lowest Calculated Quotation Shall Submit Certified True Copies Of The Following Documents Before The Approval Of The Contract: A. Philgeps Registration B. Valid Mayor’s / Business Permit C. Annual Income/ Business Tax Return (for Abcs Above P500k) D. Omnibus Sworn Statement (for Abcs Above P500k) E. Price Quotation All Particulars And Provisions Shall Be Governed By Ra 9184, And Its Implementing Rules And Regulations.
