The City Government Of Lipa Now Requests For Price Quotation For The Following Project : Name Of Project : Refill Of Fire Extinguishers For The Use Of The City Disaster Risk Reduction And Management Office Approved Budget For The Contract : ₱99,450.00 Source Of Fund : Cy 2024 Appropriation - 5% Calamity Fund The City Government Of Lipa Now Calls Interested Suppliers/service Providers For The Submission Of Their Quotation On Or Before August 19, 2024, 8:00 Am. The Supplier With The Lowest Calculated Quotation Shall Submit Certified True Copies Of The Following Documents Before The Approval Of The Contract: A. Philgeps Registration B. Valid Mayor’s / Business Permit C. Annual Income/ Business Tax Return (for Abcs Above P500k) D. Omnibus Sworn Statement (for Abcs Above P500k) E. Price Quotation All Particulars And Provisions Shall Be Governed By Ra 9184, And Its Implementing Rules And Regulations.
