The Caraga Regional Hospital Surigao City Intends To Procure Design, Build, Supply, Delivery, Installation, Testing And Commissioning Of A New Enterprise Resource Planning System / Integrated Hospital Information System With An Approved Budget For The Contract (abc) Of Fifteen Million Pesos ( Php 15,000,000.00). 2. The Caraga Regional Hospital Bids And Awards Committee (crh-bac) Now Invites Technically, Legally, And Financially Capable Suppliers For The Said Project. 3. The Procurement Procedure For This Requirement Is Negotiated Procurement-two-failed Biddings Pursuant To Section 53.1 Of 2016 Revised Implementing Rules And Regulations (irr) Of Republic Act (ra) No. 9184. The Selection Of The Successful Offer Shall Be Based On The Best And Final Offer That Will Be Submitted On The Set Deadline By The Bac And Which Would Meet The Minimum Technical Specifications Required. 4. The Interested Bidders May Obtain Further Information From The Crh Bac Secretariat From October 21, 2024 To November 12, 2024, Monday To Friday, At 8:00am To 5:00pm At Bac Secretariat Office, Caraga Regional Hospital, Surigao City. 5. The Crh-bac Will Hold A Negotiated Conference On October 29, 2024 At Exactly 9:00 A.m. Via Zoom Platform With This Link: Https://us06web.zoom.us/j/84912944975?pwd=6dvb4ztcf6xtb0jnl581cmgzqevaeq.1 And Which Shall Be Open To Prospective Bidders. 6. The Deadline For The Submission Of Bids Is Set On November 13, 2024 At 10:00 A.m. And On The Same Day The Opening Of Bids Will Follow At Exactly 2:00 P.m. The Link For The Opening Of Bids Is Only Available To All Qualified Bidders Who Procure The Bidding Documents. Late Submission Of Bids Shall Not Be Accepted. 7. The Opening And Receipt Of Bids Is Strictly Implemented Through Electronic Submission. Given The Covid-19 Pandemic, Procurement Activity Has Been Expedient And One Of This Is The Conduct Of Electronic Submission Of Bids. Interested Bidders Shall Submit The Following Documents Via Electronic Submission Of Bids Which Guidelines Are Part Of The Bidding Documents To Be Procured By The Bidder To Wit: Item No. Requirements Checklist Legal Documents 1 Philgeps Certification (platinum) During Post-qualification, The Mayor’s Permit, Sec/dti/dole And Tax Clearance, And Audited Financial Statement Will All Be Verified. An Absolute Disqualification Of The Bidder Will Take Place If One Of These Requirements Is Expired Or Not Valid Or Do Not Meet The Minimum Requirements Provided In Ra 9184. Technical Documents 2 Statement Of All On-going Contracts The Statement Of All Ongoing Contracts Shall Indicate The Following: (a) Name Of The Contract; (b) Date Of The Contract; I Contract Duration (d) Owner’s Name And Address; I Kinds Of Goods; (f) Amount Of Contract And Value Of Outstanding Contracts; And (g) Date Of Delivery. Statement Of The Prospective Bidder Of All Its On-going Government And Private Contracts, Including Contracts Awarded But Not Yet Started, If Any, Whether Similar Or Not Similar In Nature And Complexity To The Contract To Be Bid. (sec. 23.1(a)(iv), Irr Of Ra 9184) 3 Statement Of Single Largest Completed Contract (slcc) Slcc Shall Indicate, For Each Contract, The Following: (1) Name Of The Contract; (2) Date Of The Contract; (3) Contract Duration; (4) Owner’s Name And Address; (5) Kinds Of Goods; (6) Amount Of Completed Contracts, Adjusted By The Bidder To Current Prices Using Psa’s Consumer Price Index, If Necessary For The Purpose Of Meeting The Slcc Requirement (7) Date Of Delivery; And (8) End User’s Acceptance Or Official Receipt(s) Or Sales Invoice* Issued For The Contract, If Completed, Which Shall Be Attached To The Statements. Section 23.4.1.3. (ra 9184) The Bidder Must Have Completed A Single Contract That Is Similar To This Project Or Installation To An At Least Level Ii Doh Licensed Hospital With At Least 100 Authorized Bed Capacity, Equivalent To At Least Fifty Percent (50%) Of The Abc. Slcc Must Be Within 5 Years From The Submission Of Bids. For Procurement Where The Procuring Entity Has Determined, After The Conduct Of Market Research, That Imposition Of Either (a) Or (b) Will Likely Result To Failure Of Bidding Or Monopoly That Will Defeat The Purpose Of Public Bidding: The Bidder Should Comply With The Following Requirements: A. Completed At Least Two (2) Similar Contracts, The Aggregate Amount Of Which Should Be Equivalent To At Least Fifty Percent (50%) In The Case Of Non-expendable Supplies And Services Or Twenty-five Percent (25%) In The Case Of Expendable Supplies] Of The Abc For This Project; And B. The Largest Of These Similar Contracts Must Be Equivalent To At Least Half Of The Percentage Of The Abc As Required Above. Financial Documents 4 Net Financial Contracting Capacity / Committed Line Of Credit (clc) The Prospective Bidder’s Computation For Its Net Financial Contracting Capacity (nfcc) Or Clc Committed Line Of Credit (note: Nfcc Must Be Equal To Abc / Clc 10% Of Abc) 5 Joint Venture Agreement ( Jva ) If Applicable, Duly Signed Joint Venture Agreement (jva) In Accordance With Ra No. 4566 And Its Irr In Case The Joint Venture Is Already In Existence Or Duly Notarized Statements From All The Potential Joint Venture Partners Stating That They Will Enter Into And Abide By The Provisions Of The Jva In The Instance That The Bid Is Successful. Other Conditions Of The Contract Shall Be Governed By The Implementation Of The Rules And Regulations Of Ra 9184 And Other Related And Applicable Laws. 12.
