Description I. Description Supply Of Labor And Materials For The Rectification/replacement Of Various Drain Pipes And Sewerage Pipes Of Industry And Investments Building. Ii. Objective A. To Replace Old, Damaged And Corroded Drain And Drainage Pipes Which Caused Frequent Leaks And Foul Odor At The 4th Floor Male Comfort Room, Ceiling At Basement Ramp And Canteen. The Replacement Of The Pipes Will Prevent Further And Significant Damage To The Boi Structure; And B. To Protect The Health And Safety Of Boi Personnel And Guest As The Leak Emits Foul Odor And May Cause Wet Flooring. Iii. Approved Budget For The Contract The Approved Budget For The Contract Is One Hundred Thirty Thousand Pesos (php 130,000.00) Inclusive Of Vat. Iv. Scope Of Work The Contractor Shall Undertake The Following: A. Water Leak At 4th Floor Male Comfort Room 1. Mobilization And Demobilization Of Tools And Materials. 2. Removal Of Existing Ceiling At Male Comfort Room Of 3rd Floor. 3. De-clogging Of Sewerage And Drain Pipes. 4. Replacement Of Old And Busted Pipes And Connectors By Using Pvc Pipes. (all Pipes Must Be The Same Size As The Existing Sewerage And Drain Pipes And Passed The Standard By Bureau Of Product Standard Of The Philippines (bps)) 5. Inspect All Joints And Fittings For Possible Leak. 6. Testing And Observation Of Any Possible Leak. 5. Restoration Of Ceiling Of 3rd Floor Male Comfort Room. 6. Painting Of Ceiling Using Odorless Paint. 7. Cleaning And Hauling Of Materials/debris. B. Water Leak At The Basement Ramp 1. Removal Of Existing Pipes And Connectors. 2. Removal Of Existing Floor Tiles At The Dips Comfort Room Located At The Ground Floor. 3. De-clogging Sewerage/drain Pipes. 4. Replacement And Re-installation Of New Set Pvc Pipes And Connectors. (all Pipes Must Be The Same Size As The Existing Sewerage And Drain Pipes And Passed The Standard By Bureau Of Product Standard Of The Philippines (bps)) 5. Installation Of Metal Brackets For Support On The Installed Pvc Pipes To Avoid Misalignment Of Pipes. 6. Inspect And Testing Of All Joints And Fittings For Possible Leak. 7. Repair And Rectification Of Damage Area. 8. Painting Of Pipes And Affected Ceiling. C. Water Leak At The Canteen Pantry 1. Removal Of Existing Ceiling At The Canteen. 2. Removal Of Existing Floor Tiles Of The Wash Tub Basin Of Female Comfort At The Ground Floor. 3. Waterproofing Of The Wash Tub Basin. 4. De-clogging Of Sewerage And Drain Pipes. 5. Replacement Of Sewerage And Drain Pipes (if Necessary). (all Pipes Must Be The Same Size As The Existing Sewerage And Drain Pipes And Passed The Standard By Bureau Of Product Standard Of The Philippines (bps)) 6. Installation Of New Tiles For Wash Tub Basin. 7. Restoration Of Canteen Ceiling. 8. Painting Of Ceiling And Affected Area. 9. Cleaning/hauling Of Waste Materials. 10. Turn-over. Note: Please Submit Bid Proposal Together With The Technical Specification Indicating Compliance Or Conforme On Each Item. Otherwise, The Bidder Is Disqualified. V. Documents For Submission • Mayor’s Permit • Philgeps Registratrion • Price Quotation With The Scope Of Work Vi. Terms Of Payments Payment Shall Be Made Upon Full Completion Of The Above Scope Of Work Duly Certified And Accepted By The End User. Payment Shall Be Made Through Landbank’s Lddap-ada/bank Transfer Facility, Within Fifteen (15) Days After Submission Of Billing And User Acceptance Of The Product /service. Bank Transfer Fee Shall Be Charged Against The Creditor’s Account If Any. Vii. Warranty A Warranty Security Shall Be Required From The Contract Awardee For A Minimum Period Of Six (6) Months. The Obligation For The Warranty Shall Be Covered By Either Retention Money In An Amount Equivalent To At Least One (1%) Percent Of Every Progress Payment Or A Special Bond Guarantee Equivalent To At Least One (1%) Of The Total Contract Price. The Said Amount Shall Only Be Released After The Lapse Of The Warranty Period. Viii. Liabilities The Contractor Shall Be Liable To Pay The Damages In Case Of Death Or Injuries Incurred By Officials, Employees And Guest Including Damage Or Loss Of Boi Property Due To Its Fault Or Negligence During The Performance Of Civil Works At The Industry And Investments Building Ix. Contract Duration The Project Must Be Completed Within The Period Of Forty-five (45) Working Days From Receipt Of Notice To Proceed. Work Should Be Undertaken By The Contractor After Office Hours And During Weekends Only. X. Liquidated Damages If The Contractor Fails To Satisfactorily Deliver Any Or All Of The Goods And/or Perform Services Within The Period Specified In The Contract Inclusive Of Duly Granted Time Extension If Any, The Boi Shall Impose Liquidated Damages At Least Equal To One-tenth Of One Percent (0.001) Of The Cost Of The Unperformed Portion For Every Day Of Delay. Once The Cumulative Amount Of Liquidated Damages Reaches Ten Percent (10%) Of The Amount Of The Contract, The Boi May Rescind Or Terminate The Contract, Without Prejudice To Other Courses Of Action And Remedies Available Under The Circumstances.
