Description I. Description The Boi Requires An Effective And Credible Termite Treatment Provider For The Duration Of One (1) Year Which Will Cover The Boi Building. Ii. Approved Budget Of The Contract The Approved Budget For The Contract Is One Hundred Thirty Thousand Pesos (php 130,000.00) Inclusive Of Vat. Iii. Scope Of Work The Service Provider Shall Undertake The Following: 1. Conduct Termite Treatment By Baiting System, Wood Drenching And Termite Abatement Covering The Whole Building From Basement To Penthouse For Duration Of One (1) Month. • Penthouse Ceiling And Outdoor Perimeter • Walls • Door Jambs • Doors • Ac Outlets • Switch Boxes • Flooring • Ground Floor Perimeter; • And Other Areas Where Termites Might Be Active 2. Conduct Twice A Month Inspection Monitoring For Termite Activity And Gradually Reduce The Inspection Schedule Once Termite Feeding Had Slowed Down Until The Termite Infestation Is Controlled. 3. After The Termite Infestation Is Controlled, Quarterly Inspection Of The Entire Facility Will Be Done To Ensure The Termite Infestation Will Be Controlled Immediately Once Termite Infestation Reappears. 4. Remind Boi Of The Required Conditions And Standard Operating Procedures Before, During And After Treatment; And 5. Ensure Compliance With The Following: A. Complete Ppe And Other Protective Gears; B. All Chemicals To Be Used Are Fda Approved. Note: The Bidders Shall Submit The Above Scope Of Work With Indication Of Compliance With Each Item Stated Therein. Otherwise, The Bidder Shall Be Disqualified. Iv. Liability Of Service Provider In Case Of Damage To Boi Property Or Injury To Boi Personnel The Service Provider Shall Be Liable To Pay The Cost Of Actual Damage In Case Of Damage To Or Loss Of Property Including Injuries To Its Personnel During Treatment. V. Documents For Submission • Mayor’s Permit • Company Profile • Documents That Prove At Least Ten (10) Years Of Experience In The Business • List Of Clients For The Five (5) Years • Updated Certificate Of Registration To Dbm-ps Philgeps • Income/business Tax Return • Fda Certificate Of Product Registration • Fertilizer And Pesticide Authority (fpa) Training License Vi. Terms Of Payment Boi Shall Pay The Service Provider Within Fifteen (15) Working Days Upon Submission Of The Billing Statement/invoice. Vii. Contract Termini\ation The Boi Reserves The Right To Terminate The Contract In The Whole Or In Part In Case Of Delay And Failure To Perform Its Obligation Specified In The Service Contract. Viii. Contract Duration The Contract Shall Be Effective For A Period Of One (1) Year Contract Duration From Receipt Of The Notice To Proceed Subject To Automatic Extension/renewal For A Period Of Three (3) Months From Expiration Of The Contract Until A Prospective Bidder Has Been Declared As The Winning Bidder. Ix. Reservation Clause The Boi Reserves The Right To Reject Documents That Do Not Comply With The Requirements, Waive Any Formalities Of Documents Or Consider Any Submission Of Documents As Substantial Compliance Reject Any And All Bids; Declare A Failure Of Bidding, Annul The Bidding Process, Or Not To Award The Contract, Without Thereby Incurring Any Liability To The Affected Bidder Or Bidders Or If The Funds/allotment For The Program/project/activity Has Been Withheld Or Reduce Through No Fault Of The Boi.
