Registration (department Of Health/bfad) * Sketch Of The Company Location And 4r Size Photos Of The Office * Certificate Of Calibration Of The Testtool Equipment With Data Procedures * Expired Certificate Not Valid * If An Item Specified Above Is Found Out To Be Condemned Or Under Repair, The Provider Will Have To Calibrate Another Item (either The Same Item Or An Item Of Equal Calibration Cost) To Compensate The No. Of Items To Be Calibrated In The Contract. * During Actual Calibration, If The End User Would Request To Calibrate An Equipment Or Instrument Not Listed Above, The Provider And The End User Would Have To Come Up With An Agreement In Order To Offset The Details Of The Contract, Provided That The New Total Cost Would Not Go Beyond The Approved Total Cost Of The Original Contract. * Provider/ Calibrator Must Repair Or Adjust The Minor Problems Of The Equipment. * Service Provider Must Have A History Of Attending/providing Training Session/entitled "medical Equipment Training Program" With Training Certificate * Service Provider Must Have Qualified Trainor That Have History Of Medical Mequipment Training Conducted "provide Certificate Of Qualified Trainor" * Provider/ Calibrator Must Conduct A Training And Orientation On Calibration And Preventive Maintenance For 25 Participants And Provide The Following: O Certificate Of Completion For The Participants O Training And Orientation Materials O Lunch And Snacks For The 2 Days Training O All Engineers, Technician & Trainor Must Undergo Swab Test Or Vaccinated * Provider/ Calibrator Is Renewable On The Next Year Contract Depends On There Performance * After Completion Of Contract, The Provider/calibrator Must Provide: O Certificate Of Calibration Note: Binding Of Documents With Tabbing Or Label O Service
