8. Scanned copy of proof of valid registration/renewal of registration and Tax Identification Number
9. Performance statement indicating details of stores, name of the ordering Deptt, quantity ordered, Delivery Period, and quantity and dates of supplies made
10. Inspection notes in support of supplies made
11. Contract order/supply order to support experience of similar or same nature of work in the same region.
