Mdr-tb Help Desk Consultant & 1. Background Who Regional Office For The Western Pacific Via The Regional Green Light Committee (rglc) Initiative In Collaboration With Who Country Offices In Fiji (who Division For Technical Support To The Pacific) And Solomon Islands Has Coordinated Technical Assistance To The Above-mentioned Countries In Dr-tb Management For Several Years. All These Countries Are Recipients Of The Global Fund Grants Until December 2026. The Burden Of Multi-drug Resistant/rifampicin-resistant Tb (mdr/rr-tb) In The Pacific Is Low; Since 2004, 90 Mdr/rr-tb Cases Have Been Notified. On An Annual Basis, 10 Mdr/rr-tb Cases Are Reported From Pitcs. To Support Pitcs, The Pacific Dr-tb Network Was Established In 2012. Pacific Dr-tb Network Comprises The Patlab Initiative, The Mdr-tb Help Desk, And The Regional Second-line Drugs Stockpiling Mechanism. The Mdr-tb Help Desk Is A Multi-disciplinary Team That Provides Technical Assistance To Countries In The Clinical Management Of Dr-tb Cases And Has Been Functional Since 2012. It Is Composed Of The Dr-tb Clinical Expert, Representatives Of Tb Reference Laboratories, And Who Staff. Who Staff Coordinate This Group And Provide The Secretariat Support. Whenever A New Dr-tb Is Identified, This Case Is Submitted To The Mdr-tb Help Desk And Later A Patient Management Plan Is Developed Including The Patient Treatment Regimen, Second-line Drug Procurement, A Monitoring Protocol, Infection Control Aspects, And The Identification And Assessment And Prevention Of Dr-tb Of Close Contacts And Their Follow-up. External Technical Support Is Ongoing Throughout The Treatment Period. 2. Purpose/specific Objective Of The Activity To Provide Technical Assistance On Clinical Management Of Newly Diagnosed Patients Of Drug-resistant Tuberculosis (dr-tb) Throughout The Treatment Duration In 13 Pacific Island Countries And Territories (picts)[1] 3. Description Of Activities To Be Carried Out (aligned To Draft Monthly Plans) In Collaboration With The Technical Staff From Who, The Consultant Will Perform The Following Functions: - To Assist With The Management Of Current And Future Dr-tb Cases Through: O Attendance At Teleconferences With National Tb Programme (ntp) Staff And Clinicians In Picts And Submit A Summary Note To The Who Technical Officers In Countries On The Outcomes Of The Teleconference And Related Action Points; O Participating In Creating An Individualised Treatment And Monitoring Plan That Is Developed And Tailored To A Dr-tb Case In The Picts; O Support In The Design Of Second-line Drug Treatment Regimens Clearly Defining The Drugs, Dosages, Storage, Possible Side Effects, And Management; O Assist With Infection Control And Contact Tracing, Planning And Management Of The Contacts Including Dr-tb Prevention Of Eligible; And O Support In Managing Side Effects Due To Slds Through Email And/or Phone. - To Provide Interim Reports On Any New Cases And Quarterly Progress Reports Of Existing Cases - To Maintain The Electronic Dr-tb Register (redcap Database) That Stores The Case And Outcome Data For All Cases In Picts - To Participate In The Development Of The Standard Operating Procedures To Manage Dr-tb In The Respective Countries - To Discuss Periodically With Other Committee Members And Who On Certain Cases, Planning And Monitoring Of The Desk Activities Output/s Output: Technical Assistance Provided Deliverable 1.1. Workplan Deliverable 1.2. Seven Quarterly Summaries/tables On Patient Identification, Progress Outcome (3 In 2024, 4 In 2025), And Contact Investigation Deliverable 1.3. Two Annual Reports (2024 And 2025) On Ta Provided To Countries 4. Methods To Carry Out The Activity Liaise With Who Tb Technical Officers In Respective Countries, With The Ntps And Tb Doctors, And When Necessary, With Other Partners Via Emails Or Conducting Teleconferences As Needed Desktop Preparation Of Treatment Plans And Documents To Assist In Clinical Management Desktop Preparation Of Teleconferences/meeting Minutes About Clinical Management Of Cases And Contacts Desktop Inputs/contributions To Who Documents/presentations On Mdr-tb In The Pacific And Solomon Islands 5.
