Department Of Veterans Affairs va Rocky Mountain Network (visn19) performance Work Statement (pws) â regulated Medical Waste, Trace Chemotherapy, Non-hazardous Pharmaceutical, And Pathological Waste Removal 1.0. Introduction the Va Rocky Mountain Network Is Located In Glendale, Colorado, And Was Established In March 1996 As One Of 21 Veterans Integrated Service Networks (visns) In The Veterans Health Administration. The Va Rocky Mountain Network Is Visn 19 And Functions As The Responsible Structure For The Planning, Budgeting, And Service Delivery In The Provision Of Healthcare To Veterans Within Our Geographic Region. visn 19 Spans A Geographic Area Of 540,000 Square Miles Across 10 States And Is The Largest Visn In Terms Of Geographic Area In The 48 Contiguous States. Visn 19 Serves An Area Covering The States Of Utah, Montana, Wyoming, Colorado, Oklahoma And Portions Of Idaho, Kansas, Nebraska, Nevada And North Dakota. The Topography Includes The Rocky Mountains, Arid Regions And Areas Subject To Extreme Winter Weather. Significant Travel Distances Exist Between Va Healthcare Facilities Throughout Visn 19. It Is Estimated Over 1,000,000 Veterans Reside Within Visn 19's Geographic Boundaries. 2.0. Scope Of Work â contract To Provide Services For Eight (8) Va Medical Centers (vamc) And Approximately Seventy-six (76) Additional Sites Of Care To Include Community Based Outpatient Clinics (cboc), Community Living Centers (clc), Primary Care Tele-outreach Clinics (pctocs) And Healthcare Centers (hcc) Located In Visn 19, In Accordance With Current Federal, State And Local Regulations. This Will Include Any Proposed Or Newly Constructed Sites That May Be Included After The Award. The Contractor Shall Provide All Labor, Supervision, Equipment, Training, And Protective Equipment To Properly Pickup And Dispose Regulated Medical Waste (rmw) To Include Trace Chemotherapy, Non-hazardous Pharmaceuticals, And Pathological Wastes With All Federal (including Va Regulations), State, And Local Regulations For The Below Listed Location(s) Of Performance. this Will Be Awarded As A 5-year Idiq (indefinite Delivery Indefinite Quantity) Contract That Will Run Five (5) Years From Date Of Award To 12/31/2029. Minimum Guarantee For This 5-year Idiq Is The Disposal Of 200 Tons Of Waste Per Year. Maximum For This 5-year Idiq Is The Disposal Of 500 Tons Of Waste Per Year. The Government Reserves The Right To Award One Or More Contracts Resulting From This Solicitation. unit Pricing Must Include: All Labor/personnel, Any 96-gallon Reusable Roll-offs, Bags, Labeling, Manifest, Transportation, Treatment, And Disposal At An Approved Facility In Accordance With All Federal, State And Local Regulations. 2.1 Definitions regulated Medical Waste (rmw): Liquid Or Semi-liquid Blood Or Other Potentially Infectious Materials; Contaminated Items That Would Release Blood Or Other Potentially Infectious Materials In A Liquid Or Semi-liquid State If Compressed; Items That Are Caked With Dried Blood Or Other Potentially Infectious Materials And Are Capable Of Releasing These Materials During Handling; Contaminated Sharps; And Pathological And Microbiological Wastes Containing Blood Or Other Potentially Infectious Materials. other Potentially Infectious Materials: (opi) Means; the Following Human Body Fluids: Semen, Vaginal Secretions, Cerebrospinal Fluid, Synovial Fluid, Pleural Fluid, Pericardial Fluid, Peritoneal Fluid, Amniotic Fluid, Saliva In Dental Procedures, Any Body Fluid That Is Visibly Contaminated With Blood, And All Body Fluids In Situations Where It Is Difficult Or Impossible To Differentiate Between Body Fluids; any Unfixed Tissue Or Organ (other Than Intact Skin) From A Human (living Or Dead); And hiv-containing Cell Or Tissue Cultures, Organ Cultures, And Hiv- Or Hbv-containing Culture Medium Or Other Solutions; And Blood, Organs, Or Other Tissues From Experimental Animals Infected With Hiv Or Hbvopi. sharps Means Any Contaminated Object That Can Penetrate The Skin Including, But Not Limited To, Needles, Scalpels, Broken Glass, Broken Capillary Tubes, And Exposed Ends Of Dental Wires pathological Waste Consists Of Tissues, Organs, Body Parts, Human Fetuses And Animal Carcasses, Blood, And Body Fluids And Must Be Treated Through Low Level Incineration. trace Chemotherapy Waste: Are Non-hazardous Pharmaceuticals That Are Less Than 3% Volume Of A National Institute For Occupatoinal Safety And Health (nisoh) Niosh Page Table 1 Hazardous Drug Or A Facility Determined Hazardous Drug Per The U.s. Pharmacopeia (usp) 800 Standard In It S Orgininal Container, Iv-solutions Or Other Adiministrative Methods And The Personal Protective Equipment Used To Administer The Drug. It Also Includes Soiled Chux Pads And Diapers Of Patients Receiving Chemotherapy From Start Of Therapy To 48 Hours After Last Dose. non-hazardous Pharmaceutical Waste: Medications That Do Not Meet The Epa Definition Of A Hazardous Waste In In §266.500. Includes Over-the-counter Pharmaceutical, Dietary Supplement, Or Homeopathic Drugs Which Are Not A Solid Waste, As Defined In §261.2, And Therefore Not A Hazardous Waste Pharmaceutical, If It Has A Reasonable Expectation Of Being Legitimately Used/reused (e.g., Lawfully Redistributed For Its Intended Purpose) Or Reclaimed. Forms May Include Vials, Syringes, Pills, And And Needles Assoricated With It S Administration. 2.2. Medical Waste Requirements the Contractor Shall Provide Labor, Transportation, Collection, Removal, Treatment And Destruction For The Disposal Of All Rmw Generated By The Listed Visn19 Medical Centers, Their Cbocs, Clcs, Hhcs And Pctocs As Specified In This Pws And Cost/price Schedule In Accordance With The Terms And Conditions Of This Contract. Performance Will Be According To The Requirements Contained In This Pws. services Shall Also Include The Option For Mail-in Disposal From Rural Clinics. This Service Shall Provide All Required Labeling And Packaging. vamcs, Cbocs, Clcs, Hhcs, And Pctocs Will Be Responsible For Properly And Lawfully Filling And Storing The Rmw. Vamcs, Cbocs, And Pctocs Shall Store The Full Rmw In Containers Supplied And Approved By The Contactor. Infectious Medical Waste Containers Shall Be Leak Proof, Have Tight-fitting Covers, And Be Kept Clean And In Good Repair. The Storage Of Rmw Shall Be Confined To An Area On The Premises To Which Only Authorized Personnel Shall Have Access. visn 19 Facilities Also Require The Disposal Of Wastes The Require Low-grade Incineration To Include: trace Chemotherapy non-hazardous Pharmaceuticals pathological Waste, Including Animal Carcasses all Work Shall Be Accomplished In Accordance With Va, Federal, State, And Local Regulations. The Contactor Shall Not Pick-up Medical Waste Outside Of The Designated Hours. The Scheduled Pick-up Times Will Be Mutually Agreeable To Both Parties. The Contractor Is Responsible For Notifying The Appropriate Government Official At Each Site For Arrangements For The Pick-up Of The Rmw. If Routine Pick-up Is Scheduled On A Recognized Government Holiday, Then The Pick-up Must Be Rescheduled And Picked-up Either Within Three (3) Days Before Or After The Holiday. Contractor Requirements And Qualifications 3.1. The Contractor Shall: provide And Maintain All Licenses And Permits For Operational Personnel, Trailers, Containers, Vehicles And Other Resources Required For Proper Removal Of Biohazardous Waste In Accordance With All Applicable Federal, State, Municipal, And Local Regulations (e.g. Osha, Epa, Mt/wy/ut/ok Deq, Co Dphe, 49 Cfr (dot) Including, But Not Limited To Hm-18). the Contractor Shall Provide The Services Enumerated Above At The Rates Set Forth Which Includes All Applicable Taxes, Assessments, And Related Charges, Including Adjustments For It. The Contractor Shall Provide All The Necessary Equipment To Perform Such Services And All Such Equipment Shall Comply With All Applicable Federal, State, County, And Municipal Laws, Regulations, Guidelines And Permits. the Contractor Shall Provide A Variety Of Overpack Containers Sizes With Liners. All Containers And Liners Will Be Labeled According To All Federal, State And Local Regulations. Any Changes To Type, Size, Amount Of Equipment, And The Frequency Of Service May Be Mutually Agreed To In Writing By Both Parties Without Affecting The Validity Of The Contract. supply All Overpack Containers For Transport Of The Above Waste And Perform All Labeling, Manifesting, Labor, Transportation, Proper Disposal And Certificate Of Destruction For Each Pick Up Of Waste In Accordance With All Federal, State And Local Requirements. contain Any Spills Which Occur While The Medical Waste Is Being Handled, Removed, Or Transported By The Contractor. Any Clean-up Shall Be The Responsibility Of The Contractor. immediately Notify All Appropriate Authority Having Jurisdiction (ahj) And The Contracting Officer S Representative (cor) Should A Vehicle Be Involved In A Spill Or Accident That Renders The Vehicle Non-compliant With Applicable Rules And Regulations. ensure All Spillage Or Leaks Are Promptly Cleaned-up Along Any Transportation Route If Such Spillage Or Leaks Are From The Contractor S Hauling Equipment And/or Vehicles. not Allow Any Rmw To Be Compacted Before Being Sterilized. ensure All Waste Overpack Containers Are Cleaned And Disinfected For Use When Delivered To The Facility. ensure All Waste Containers Are Rigid And Puncture Resistant And Labeled With The Word Biohazard . ensure All Chemotherapy Waste, Non-hazardous Pharmaceuticals, And Pathological Are Picked Up During Rmw Pickups And Are Incinerated In A Permitted Treatment Facility. equip Transportation Vehicles With Fully Enclosed Leak-resistant Cargo Body Maintained In Sanitary Condition And Secured When Left Unattended. Vehicles Shall Be Properly Disinfected As A Result Of Leaks Or Spills. ensure Rmw Is Not Combined With Other Waste. ensure Transporter Sign, Date, And Furnish A Copy Of The Manifest Prior To The Rmw Leaving The Pickup Site. Certify That The Manifest Data Matches The Load To Be Removed. Deliver The Waste To An Authorized Facility And Sign The Manifest To Verify That The Regulated Material Has Been Delivered. Return All Remaining Copies Of The Manifest To The Generator, Assuring Final Disposal. supply Up To Twenty-five (25) 96-gallon Internal Transport Containers Per Main Hospital Waste As Outlined In Schedule Section. provide A Monthly Summary Report Of All Rmw Disposed To The Vamc Cor Within 14 Days Of The Final Pick-up In That Calendar Month. Report Shall List Each Waste Stream; Regulated Medical, Trace Chemotherapy, Non-hazardous Pharmaceuticals, And Pathological, With Location Totals. the Contractor Shall Lead A Quarterly Program Meeting With The Co And The Cor S To Address Contract Performance. These Meetings May Take Place Virtually With All Stations In Attendance. Should Contract Deficiencies Arise, Meetings May Be Held On A More Frequent Basis To Resolve Any/all Issues. The Contractor Shall Provide Draft Meeting Minutes To The Cor For Review Within Seven (7) Calendar Days Of The Meeting And Final Minutes Within Seven (7) Calendar Days Of The Cor S Draft Review. No Containers Will Be Picked Up That Are Wet Or Leaking. If The Contractor Fails Or Refused To Correct Deficiencies Promptly, The Contractor Officer May Issue An Order Stopping All Or Any Part Of The Work And Hold The Contractor In Default Of The Contract. the Contractor Shall Comply With Applicable Federal, State And Local Safety And Fire Regulations And Codes That Are In Effect At The Beginning Of The Contract Period. The Contractor Shall Keep Abreast Of, And Comply With, Changes In These Regulations And Codes Applicable To The Contract. the Contractor Is To Provide Copies Of All Safety Data Sheet (sds) To The Cor If Chemical Products Used By The Contractor Are Brought On Site. 4.2. Spill Handling the Contractor Shall Report All Spills To The Facility S On-site Poc And To The Cor. The Contractor Shall Be Legally And Financially Responsible For The Cost Of All Clean-up Of Spills Created By The Contractor. The Contractor Shall Be Required To Clean-up Any Spills Created During The Collection Process Utilizing Non-va Resources. 4.3. Recordkeeping in The Performance Of All Services To Be Provided Hereunder, The Contractor Agrees To Comply With All Applicable Permits, All Applicable Federal, State, County, And Municipal Laws And Ordinances And All Lawful Orders, Rules, Regulations And Guidelines Of Any Duly Constituted Authority, Including But Not Limited To Social Security, Income Tax Withholding Laws, Unemployment Compensation Laws, Environment, Safety And Health Laws And Manifesting Requirements. the Contractor And Its Affiliates Shall Keep And Retain Adequate Books And Records And Other Documentation Including Personnel Records, Correspondence, Instructions, Plans, Receipts, Vouchers, Copies Of All Manifests And Any Other Records Or Repots Or Memoranda, Consistent With And For The Period Required By Applicable Regulatory Requirements And Guidelines Pertaining To Performance Of The Services. contractor Shall Provide An Electronic Manifest Database As A Redundancy For All Stations To Access. 5.0. Contract Performance Monitoring monitoring Of Contractor S Performance Shall Be Demonstrated Through Direct Observation. The Cor And/or The Alternate Cor Shall Be The Va Official Responsible For Verifying Contract Compliance. After Contract Award, Any Incidents Of Contractor Noncompliance As Evidenced By The Monitoring Procedures Shall Be Forwarded Immediately To The Contracting Officer. 5.1. Contractor Monitoring By Service: at The Time Of Contract Award, The Contracting Officer Will Appoint A Cor And The Alternate Cor To Assist With The Contract Monitoring Requirements.
