Note That The Terms Contracting Officer Representative (cor) And Contracting Officer Technical Representative (cotr) May Be Used Interchangeably And Refer To The Same Entity. this Project Is Located At The Veteran Affairs (va) Northport Ny Campus At 79 Middleville Road, Northport Ny 11768. The Campus Contains Approximately 268 Acres, Approximately 107 Of Those Acres Have Been Declared A Historic District. The Northport Va Campus Was Opened In 1928 As A Neuropsychiatric Veteran S Hospital And Consists Of Numerous Facilities Providing Physical Infrastructure For Patient Care, Research, Education And Administration. due To Modifications That Were Done To The Structures Over The Years, The Existing Emergency Eyewash And Shower Stations May Not Be Adequate For The Current Space Program Or May Not Be Required At All. Emergency Eyewashes And Showers Are First Aid Rather Than Preventive Measures And Must Be Readily Available In Spaces Where Occupants May Be Exposed To Hazardous Chemicals. Emergency Eyewash And Shower Stations Are Not To Be Used In Lieu Of Appropriate Personal Protective Equipment (ppe). Evaluation Of Areas For Emergency Eyewashes And Showers Was Conducted As If Ppe Were Not Used. the Scope Of This Project Is To Provide New Emergency Eyewash And Drench Shower Stations To The Required Spaces And Upgrade Or Remove Existing Stations. The Contractor Shall Furnish Labor And Materials And Perform Work For Installations, Upgrades, Or Removal Of Emergency Eyewash And Shower Stations As Shown On Contract Documents (e.g. Sow, Drawings, Schedules, Specifications). Work May Include General Demolition, Construction, Alterations, Mechanical, Electrical, & Plumbing Work, Necessary For The Successful Execution And Completion Of The Project. this Document Adheres To The Requirements Of: the Joint Commission Perspectives (march 2021) vha Directive 7704: Emergency Eyewash And Shower Program (october 12, 2021) ansi Z358.1- 2014 specification 22 40 00 Section 2.4: Emergency Fixtures va Plumbing Design Manual (https://www.cfm.va.gov/til/dmanual/dmplbg.pdf) va Plumbing Specifications (https://www.cfm.va.gov/til/spec.asp#22) the Contract Completion Time Is 300 Calendar Days From Receipt Of Notice To Proceed. work Shall Be Governed By: this Statement Of Work (sow), attachment 1: Equipment Schedule attachment 2: Building Floor Plans attachment 3: Project Specifications hazard Assessments: veterans Affairs Medical Center Northport (vamc) Safety Office Has Performed Hazard Assessments, Including A Review Of The Chemicals Used, Across The Facility To Determine Where Emergency Drench Showers And Eyewashes Are Needed For Immediate Use When Exposure To Corrosive Materials Occurs. Assessments Were Completed As A Prelude To The Development Of This Procurement Package Whenever A Change In Chemicals, Processes, Or Controls Could Result In Changes Warranting The Addition Or Removal Of Emergency Drench Showers And Eyewashes. this Work May Require After-hours Or Weekend Work Implementation Due To Ongoing Service Location Processes, And/ Or Requirements For Water Supply Shutdowns Following Lockout-and Tagout Procedures In Coordination With Va Cor And Plumbing Shop. identification Of Deficiencies Are To Be Itemized Before Implementation Of Work During Walkthroughs, However, When Additional Issues Are Identified During Work Processes, Notifications Will Be Provided To The Project Cor For Resolution. Contractors And Subcontractors Are Responsible To Confirm Conditions Before The Initiation Of Work. accessibility Criteria: emergency Flushing Equipment Should Be Located: in Accessible Locations That Require No More Than 10-seconds (or 55ft) Unobstructed Travel (no Latching Doors) And On The Same Level As The Hazard. This Distance May Need To Be Reduced At Areas Where Extremely Corrosive Chemicals Are Used. in A Well-lit Area. water Flow Is Established Between 33-inches And 53-inches From The Standing Grade, And Minimum 6-inches From A Wall, Free Of Obstructions. the Eyewash Station Must Have Hands-free, Stay-open Valves (single Action Operation So That, After Actuation, Water Continues To Flow). in An Area Clear Of Any Obstruction That May Impede Employee Access. Employees Will Not Be Required To Pass Through A Locked Or Latched Doorway. worker Must Be Able To Flush Both Eyes Simultaneously For At Least 15 Minutes. in A Location Marked By Highly Visible Signage. placement And Design Must Meet Accessibility Requirements Of The Architectural Barriers Act (when Applicable), Va Barrier Free Design Guide (pg-18-13) And Latest Edition Of The Va Plumbing Design Manual. selection Criteria: only Ansi-approved Eyewashes And Drench Showers Shall Be Purchased And Installed. ansi-approved Self-contained Eyewashes Are Acceptable Upon Review And Approval By The Facility Safety Manager. portable Eyewash Stations Must Also Meet Ansi Z358.1-2014 Requirements. personal Eyewash Flushing Bottles Are Acceptable Only If The Hazard Is Biological. installation: emergency Eyewashes And Drench Showers Will Be Installed In Accordance With Manufacturer's Specifications, Va Specifications And Ansi Installation Standards. the Equipment Must Be Installed On A System Capable Of Delivering The Required Quantity And Temperature Of Water. provide Domestic Cold And Hot Water Lines For Each Emergency Eyewash/ Shower Station. Refer To Attachment #1 For Additional Requirements. provide New Isolation Valves On The Domestic Cold & Hot Water Supply Lines For Maintenance Purposes. Provisions Shall Be Made To Prevent Unauthorized Shut Off. the Tepid Water Line Shall Be Supplied By An Asse Approved Thermostatic Mixing Valve (tmv). All Emergency Eyewashes And Drench Showers Will Deliver Tepid Water (60-100 Degrees Fahrenheit). No Emergency Eyewash Or Drench Shower Will Be Connected Solely To A Hot Water Supply. Obtain Cor Approval Prior To Setting The Final Tmv Temperature Set Point. emergency Showers Shall Be Capable Of Delivering Flushing Fluid At A Minimum Of 20 Gpm For A Minimum Of 15 Minutes. Emergency Eyewashes Shall Be Capable Of Delivering Flushing Fluid To The Eyes Not Less Than 0.4 Gpm For 15 Minutes. emergency Shower Shall Provide A Flushing Fluid Column That Is At Least 82in And Not More Than 96 In. In Height From The Surface On Which A User Stands. control Valve Shall Remain Open Without The Use Of The Operator S Hands Until Intentionally Closed. all New Domestic Water Pipe, Fittings And Components Shall Be Insulated And Labeled Properly. portable Wall Hung Emergency Eyewash Units Shall Be Properly Fastened To The Wall / Structure. Confirm And Coordinate With Cor Prior To Installing For Review And Approval Of Fastening Method. removal/demolition: emergency Eyewashes And Drench Showers Determined To No Longer Be Needed Are To Be Tagged As Do Not Use , Water Supply Turned Off, Work Order Submitted For Priority Removal, And Removed From Service As Soon As Possible. Dedicated Fixture Water Supply Piping Leading To Decommissioned Emergency Eyewash And/ Or Drench Shower Units Will Be Cut Back To The Nearest Active Branch/main And Capped To Prevent Dead Legs. all Removed/ Discarded Devices Shall Remain On-campus Until Determined Otherwise By Cor. activation And Testing: 1. Engineering Service Will Maintain Emergency Eyewashes And Drench Showers In Accordance With The Manufacturer's Specifications And Ansi Z358.1. Modifications That Void The Ansi Approval Are Not Allowed. Each New And Existing Device Shall Be Fitted With A New Test Tag (from The Manufacturer). Water Preservative Solution Bottles Shall Be Administered As Required And Documented. 1.1 Nara Records Management contractor Shall Comply With All Applicable Records Management Laws And Regulations, As Well As National Archives And Records Administration (nara) Records Policies, Including But Not Limited To The Federal Records Act (44 U.s.c. Chs. 21, 29, 31, 33), Nara Regulations At 36 Cfr Chapter Xii Subchapter B, And Those Policies Associated With The Safeguarding Of Records Covered By The Privacy Act Of 1974 (5 U.s.c. 552a). These Policies Include The Preservation Of All Records, Regardless Of Form Or Characteristics, Mode Of Transmission, Or State Of Completion. in Accordance With 36 Cfr 1222.32, All Data Created For Government Use And Delivered To, Or Falling Under The Legal Control Of, The Government Are Federal Records Subject To The Provisions Of 44 U.s.c. Electronic Records And Associated Metadata Must Be Accompanied By Sufficient Technical Documentation To Permit Understanding And Use Of The Records And Data. the Department Of Veterans Affairs Northport Medical Center And Its Contractors Are Responsible For Preventing The Alienation Or Unauthorized Destruction Of Records, Including All Forms Of Mutilation. Records May Not Be Removed From The Legal Custody Of The Department Of Veterans Affairs Northport Medical Center Or Destroyed Except For In Accordance With The Provisions Of The Agency Records Schedules And With The Written Concurrence Of The Head Of The Contracting Activity. Willful And Unlawful Destruction, Damage Or Alienation Of Federal Records Is Subject To The Fines And Penalties Imposed By 18 U.s.c. 2701. In The Event Of Any Unlawful Or Accidental Removal, Defacing, Alteration, Or Destruction Of Records, Contractor Must Report To Northport V.a. The Agency Must Report Promptly To Nara In Accordance With 36 Cfr 1230. the Contractor Shall Immediately Notify The Appropriate Contracting Officer Upon Discovery Of Any Inadvertent Or Unauthorized Disclosures Of Information, Data, Documentary Materials, Records Or Equipment. Disclosure Of Non-public Information Is Limited To Authorized Personnel With A Need-to-know. The Contractor Shall Ensure That The Appropriate Personnel, Administrative, Technical, And Physical Safeguards Are Established To Ensure The Security And Confidentiality Of This Information, Data, Documentary Material, Records And/or Equipment Is Properly Protected. The Contractor Shall Not Remove Material From Government Facilities Or Systems Operated Or Maintained On The Government S Behalf, Without The Express Written Permission Of The Head Of The Contracting Activity. When Information, Data, Documentary Material, Records And/or Equipment Is No Longer Required, It Shall Be Returned To The Department Of Veterans Affairs Northport Medical Center Control, Or The Contractor Must Hold It Until Otherwise Directed. Items Returned To The Government Shall Be Hand Carried, Mailed, Emailed, Or Securely Electronically Transmitted To The Contracting Officer. Destruction Of Records Is Expressly Prohibited Unless In Accordance With Paragraph (4). the Contractor Is Required To Obtain The Contracting Officer's Approval Prior To Engaging In Any Contractual Relationship (sub-contractor) In Support Of This Contract Requiring The Disclosure Of Information, Documentary Material And/or Records Generated Under, Or Relating To, Contracts. The Contractor (and Any Sub-contractor) Is Required To Abide By Government And The Department Of Veterans Affairs Northport Medical Center Guidance For Protecting Sensitive, Proprietary Information, Classified, And Controlled Unclassified Information. the Contractor Shall Only Use Government It Equipment For Purposes Specifically Tied To Or Authorized By The Contract And In Accordance With The Department Of Veterans Affairs Northport Medical Center Policy. the Contractor Shall Not Create Or Maintain Any Records Containing Any Non-public The Department Of Veterans Affairs Northport Medical Center Information That Are Not Specifically Tied To Or Authorized By The Contract. the Contractor Shall Not Retain, Use, Sell, Or Disseminate Copies Of Any Deliverable That Contains Information Covered By The Privacy Act Of 1974 Or That Which Is Generally Protected From Public Disclosure By An Exemption To The Freedom Of Information Act. the Department Of Veterans Affairs Northport Medical Center Owns The Rights To All Data And Records Produced As Part Of This Contract. All Deliverables Under The Contract Are The Property Of The U.s. Government For Which The Department Of Veterans Affairs Northport Medical Center Shall Have Unlimited Rights To Use, Dispose Of, Or Disclose Such Data Contained Therein As It Determines To Be In The Public Interest. Any Contractor Rights In The Data Or Deliverables Must Be Identified As Required By Far 52.227-11 Through Far 52.227-20. training. All Contractor Employees Assigned To This Contract Who Create, Work With, Or Otherwise Handle Records Are Required To Take Vha-provided Records Management Training. The Contractor Is Responsible For Confirming Training Has Been Completed According To Agency Policies, Including Initial Training And Any Annual Or Refresher Training. 1.2 Information Security Privacy information Made Available To The Contractor Or Subcontractor By Va For The Performance Or Administration Of This Contract Or Information Developed By The Contractor/subcontractor In Performance Or Administration Of The Contract Shall Be Used Only For Those Purposes And Shall Not Be Used In Any Other Way Without The Prior Written Agreement Of The Va. This Clause Expressly Limits The Contractor/subcontractor's Rights To Use Data As Described In Rights In Data - General, Far 52.227-14(d) (1). the Term Security Incident Means An Event That Has, Or Could Have, Resulted In Unauthorized Access To, Loss Or Damage To Va Assets, Or Sensitive Information, Or An Action That Breaches Va Security Procedures. Information Made Available To The Contractor Or Subcontractor By Va For The Performance Or Administration Of This Contract Or Information Developed By The Contractor/ Subcontractor In Performance Or Administration Of The Contract Shall Be Used Only For Those Purposes And Shall Not Be Used In Any Other Way Without The Prior Written Agreement Of Va. 1.3 Contractor Confined Space Entry Requirements contractor Will Be Informed By The Medical Center S Cotr, (in-charge Of The Confined Space Work), If The Workplace Is A Permit-required Or Non-permit Required Confined Space, And If That Confined Space Entry Is Allowed Only Through Compliance With A Permit Space Program Meeting The Requirements Of Medical Center Memorandum 001-183 Confined Space Program. contractor Will Be Informed By The Cotr Of Any Hazards Known To The Medical Center For The Space In Question And Will Determine If A Confined Space Work Permit Is Required. contractor Will Be Apprised By The Medical Center S Cotr, (in-charge Of The Confined Space Work), Of Any Precautions Or Procedures That The Medical Center Has Implemented For The Protection Of Employees In Or Near Permit Space Where Contractor Personnel Will Be Working. the Medical Center S Cotr, (in-charge Of The Confined Space Work), Will Advise The Contractor Of How To Fill Out The Required Forms And Gain The Necessary Confined Space Entry Permits From The Va Fire Department And Be Responsible For The Contractor Following The Requirements Of Cm 001-183 Confined Space Program. the Medical Center S Cotr, (in-charge Of The Confined Space Work), And Contractor Will Coordinate Entry Operations When Both Medical Center Personnel And Contractor Personnel Will Be Working In Or Near Permit Spaces As Required By 29 Cfr 1910.146(d)(11) And 1926.21(b)(6). contractor Will Obtain Any Available Information Regarding Permit Space Hazards And Entry Operation From The Medical Center S Cotr, (in-charge Of The Confined Space Work). at The Conclusion Of The Entry Operations, The Medical Center S Cotr, (in-charge Of The Confined Space Work), And Contractor Will Discuss Any Hazards Confronted Or Created In Permit Spaces. the Contractor Is Responsible For Complying With This Medical Center Memorandum 001 183 Confined Space Program, 29 Cfr 1910.146(d) Through (g) And 1926.21(b)(6). The Medical Center Will Provide Rescue And Emergency Services Required By 29 Cfr 1910.146(k) And 1926.21(b)(6). prior To Awarding A Contract To Commence Entry Operations, The Contractor Shall Submit Proof Of Training Required Under 29 Cfr 1910.146(g) To The Medical Center Contracting Officer And The Medical Center S Cotr, (in-charge Of The Confined Space Work). questions Regarding The Medical Center S Permit-required Confined Space Entry Program Can Be Addressed To The Va Fire Department (ext. 2769) And Cotr. 1.4 Infection Control Risk Assessments (icra / Wicra) - General Notes the Vamc Requirements Shall Be Met At All Times. The Icra Status Will Be Reviewed Weekly By The Cor. Contractor Will Coordinate All Work With Cor. contractor Shall Utilize Critical Branch Power For Any Dust Mitigation Equipment, I.e., Negative Air Machine (nam). If Critical Branch Power Is Not Readily Accessible, Contractor Shall Run A Temporary Critical Circuit Back To The Nearest Critical Branch Panel. At No Time Shall There Be Extension Cords And/ Or Temporary Wiring Laid Out Within The Egress Corridors And/ Or Veteran Patient Areas. Any Temporary Wiring Shall Be Installed In The Ceiling Space. contractor Shall Maintain All Exits During Construction In Compliance With Nfpa 101 Life Safety Codes Including: nfpa 101-4.6.10 2015 Construction, Repair, And Improvement Operations. nfpa 101-7.1.10 2015 Means Of Egress Reliability. nfpa 101-18.7.9 2015 Requirements For Emergency Lighting Systems. nfpa 241 2010 Standard For Safeguarding Construction And Demolition Operations. contractor Shall Provide A Zippered Door At Entrance To The Work Area. Containment Barriers Shall Seal Work Area Air-tight. place Tacky Mats At Entrance And Exit Of Work Area. contain Construction Waste Before Transport In Tightly Covered Containers. Coordinate Access Routing Of Materials, Waste, And Personnel Through The Facility With The Cor. seal Ventilation Grills And Open Ducts In Work Area With Visqueen. contractor Shall Submit A Water Infection Control Risk Assessment (wicra) Form For Each Work Location. 2.0 Tasks Project Tasks Are As Follow: task 1: Remove Selected, Existing Emergency Eyewash And/ Or Drench Shower Units task 2: Install New Emergency Eye/face Wash, Combination Shower & Eye/face Units task 3: Upgrade Existing Emergency Eyewash And/ Or Drench Shower Units task 4: Signage And Visibility task 5: Activation, Testing, And Demonstration Of Meeting Compliance Criteria note: Contractor Must Obtain A Notice To Proceed (ntp) Prior To Performing Work And Prior To Purchasing Equipment. Contractor Shall Submit All Product Submittals And Receive Approval From Cor As A Requirement To Receiving Ntp. task 1: Remove Selected, Existing Emergency Eyewash And/or Drench Shower Units attachment #1 Identifies Emergency Eyewash And Drench Shower Stations And Accessories Which Are To Be Removed By The Contractor. The Items Listed Below Shall Also Be Performed By The Contractor: contractor Shall Coordinate With Cor Prior To Commencing Demolition Work In Order To Coordinate Shut Down With The Facility And To Obtain All Required Permits And Approval From Cor To Commence Work. contractor Will Furnish All Labor And Materials To Remove Specified Stations And To Restore Damaged Walls, Ceilings, And Floors To Original Conditions Before Continuing With Work At Other Locations. all Discarded Devices Are To Be Inspected And Approved By The Cor Prior To Being Removed Offsite By The Contractor. domestic Cold And Hot Water Lines Serving The Emergency Fixture Shall Be Isolated Prior To Device/piping Removal Or Alteration. Prior To Start Of Work, Provisions Shall Be Made To Locate And Temporarily Flag Additional Upstream Valves In Case The Main Fixture Valves Are Not Holding Once The Pipes Are Cut. This Will Greatly Reduce The Risk Of Flood Damage. emergency Fixtures That Are To Be Removed Completely Or Relocated Shall; Have The Domestic Water Piping Cut And Capped Back To Active Main/riser To Prevent Deadlegs . The Emergency Fixture Waste Piping (if Any) Shall Be Cut And Capped Back To Main Active Branch/ Riser. If Piping Extends Below Ground, Cut And Cap At Floor. All Applicable Pipe Hangers, Fittings And Appurtenances Shall Be Removed And Discarded. emergency Fixtures That Are To Be Retrofitted With Thermostatic Mixing Valves (tmv) Shall; Existing Piping, Fittings, Hangers And Components That Are To Remain, Shall Be Inspected By The Contractor And Replaced With New Wherever Deemed In Need Of Repair. faucet Mounted Eyewashes That Are Removed Shall Be Replaced With A New Laminar Flow Control That Is Compatible With The Existing Faucet. task 2: Install New Emergency Eye/face Wash, Combination Shower Eye/face Wash Units contractor Will Provide All Labor, Materials, And Supplies To Conduct A Comprehensive Inspection Of All Emergency Eyewash And Drench Showers Station Across The Facility. There Are Approximately 127 Stations, Refer To Attachment #1 For New Work Requirements. Contractor Is To Furnish All Labor And New Materials To Install The Emergency Fixture And/or New Plumbing Components As Required. contractor Shall Coordinate With Cor Prior To Commencing New Work In Order To Coordinate Shut Down With The Facility And To Obtain All Required Permits And Approval. As Part Of This Requirement, Contractor May Be Required To Establish A Work Exclusion Zone By Erecting Temporary Dust Control Barriers And Dust Control Measures As Required In Specifications And Infection Control. domestic Cold And Hot Water Lines Serving The Emergency Fixture Shall Be Isolated Prior To Device/ Piping Alteration. Prior To Start Of Work, Provisions Shall Be Made To Locate And Temporarily Flag Additional Upstream Valves In Case The Main Fixture Valves Are Not Holding Once The Pipes Are Cut. This Will Greatly Reduce The Risk Of Flood Damage. provide New Domestic Cold And Hot Water Piping To Each New Emergency Fixture, With New Ball Valves, Insulation, Labels, Hangers, Fittings, Approved Temperature Gauge And All Required Appurtenances. Provide New Waste Piping As Required, Including Trap And Venting. all New Pipe Wall Penetrations Shall Be Sealed In An Approved Manner To Maintain Wall Fire Rating. restore Damaged Walls, Ceilings, Floors, And Paint To Original Conditions Before Continuing With Work At Other Locations. new Countertop Mounted Eye/face Wash Shall Be Compatible With Existing Pre-drilled Countertop Hole. If None Exist, Contractor Shall Drill New Hole In Accordance With Manufacturer Specifications And Shall Exercise Great Care To Not Damage The Surrounding Countertop Surface Areas. Provide With New Escutcheons. Task 3 Upgrade Selected, Existing Emergency Eyewash And/ Or Drench Shower Units furnish All Labor And Materials To Upgrade Existing Eyewash And Drench Shower Systems To Achieving Z358.1-2014 Compliance For Tepid Water And Other Required Parameters. Follow The Sequential Steps Outlined In Task # 2. Attachments # 1 And # 2 Detail The Locations Where Upgrades Are To Be Implemented To Meet Osha/ Ansi Compliance. â· Replace Existing Cold And Hot Water Stop Valves With New Ones. Insulate Existing Piping To Remain And Label Accordingly. Task 4 Signage And Visibility contractor Will Provide All Labor, Materials, And Supplies To Install Signage Identifying The Location Of The Emergency Eyewash And Drench Shower Stations Identified In Attachment # 1, Which Identifies Where New Signage Is To Be Installed, And Where Existing Signage Is To Be Removed.
