Woodrow Wilson Avenue, Jackson, Mississippi 39216 Has A Requirement For A Capitated (per Member/per Month) Community Based Outpatient Clinic (cboc) To Be Within City Limits Of Kosciusko, Attala County, Mississippi. It Is Estimated That There Is A Population Of Approximately 18,267 Veterans In The Area. The Proposed Project Will Be A Five-year Fixed Price Indefinite Delivery Indefinite Quantity (idiq) Contract 4-1-2025 To 3-31-2030 And Individual Task Orders Will Be Issued On An Annual Basis. the Cboc Must Have The Necessary Professional Medical Staff, Diagnostic Testing And Treatment Capability, And Referral Arrangements Needed To Ensure Continuity Of Health Care. services To Be Provided At The Cboc: Primary Care Cboc (approx. 2120) - Offers Both Medical And Mental Health Care (physically On Site And By Telehealth) And May Offer Support Services Such As Pharmacy And Laboratory.
