Your Proposal Shall Include Original Or Certified True Copies Of The Following: 1 Certification Of Philgeps Registration Or Philgeps Registration Number1; 2 Current And Valid Mayor’s Or Business Permit Issued By The City Or Municipality Where The Principal Place Of Business Of The Prospective Bidder Is Located, Or The Equivalent Document For Exclusive Economic Zones Or Areas. Or A Recently Expired Mayor’s/business Permit Together With The Official Receipt As Proof That The Prospective Bidder Has Applied For Renewal Within The Period Prescribed By The Concerned Local Government Unit, Shall Be Accepted By The Philgeps For The Purpose Of Updating The Philgeps Certificate Of Registration And Membership In Accordance With Section 8.5.2 Of The 2016 Revised Implementing Rules And Regulations Of Republic Act No. 9184. 3 Notarized Omnibus Sworn Statement (annex C).
