Equipment Required: 1 Unit - Dump Truck, 1 Unit - One Bagger Concrete Mixer, 1 – Unit Electric Bar Cutter, 1 – Unit - Electric Bar Bender And Minor Tools (electric Grinder, Hand Drill Etc.) 2. The Original And Four (4) Copies Of The Quotation And Other Documents Must Be Placed In A Sealed Envelope And Should Be Properly Labeled On The Envelope Indicating The Name Of The Project, Contact Number, Name And Address Of The Contractor, Name And Address Of The Implementing Agency: Documentary Requirements: 1. Latest Income And Business Tax Returns 5. Notarized Omnibus Sworn Statement 6. Bid Forms/bill Of Quantities, And 7. Detailed Estimates Non-submission Of The Above Required Documents Shall Be Ground For Disqualification. 3. Forms And Plans May Be Purchased By Interested Constructors Starting September 5 To11, 2024 From The Address Below And Upon Payment Of One Thousand Pesos (php1,000.00) Vat Inclusive. 4. All Sealed Quotations Together With The Documentary Requirements Shall Be Submitted On Or Before The 9:00am, September 13, 2021 To The Bids And Award Committee For The Port Management Office Of Bicol, Regional Government Center, Rawis, Legazpi City Addressed To The Bac Chairperson. 5. Opening Of Quotation Will Immediately Follow At 10:00am Of September 13, 2024 At The Conference Room Of Pmo Bicol. Late Bids Shall Not Be Accepted. 6.
