However, It Is Imperative That All Specified Certifications: Itil, Iso, Cdcp, And Ccp Are Covered. • Plm Will Inspect Installation In Progress. It Is The Responsibility Of The Provider To Schedule Regular And Milestone Inspection Times With Plm. It Is Incumbent Upon The Provider To Verify That The Installation Is Inspected By Plm. • Hardware To Be Delivered Must Be Compatible To Lot 1 – Sip Trunk Service • The Provider’s Technical Section, Through Its Relationship Manager, Shall Meet With Plm-icto At A Minimum Of Once A Month, Or Whenever Necessary, To Discuss Operational Concerns, If Any. The Plm-icto May Also From Time-to-time Request For Assistance In Planning Expansion Projects. • Certificate Of Guarantee By The Provider On Its Capacity To Fulfill The Other Items Under The Inclusions/service Level Agreement [pls. Itemize Such Items In The Certification] Guarantees • The Provider Shall Guarantee That The Entire Work Is Free From All Defective Workmanship And Materials. • The Provider Shall Indemnify And Save Harmless Plm From And Against All Liability For Damages Arising From Injuries Or Disabilities To Persons Or Damages To Property Occasioned By Any Commission Or Omission Of The Provider Or Any Of Its Subcontractors. • The Provider Must Guarantee The Installation Of License Within The Specified Period And Location Of Delivery. • The Provider Must Submit A Certification That All Items To Be Delivered Will Cover The One (1) Year Warranty Period. The Certification Must Be An Authorized Person Of The Provider. • The Provider Must Bear All The Expenses In The Delivery And Installation Of The Licenses And All Equipment. • The Provider Must Also Conduct Monthly Visits For Preventive Maintenance. • License/s Must Be Perpetual. • The Winning Provider Must Warranty The Merchantability Of The Products. • The Provider Services Are Valid For One (1) Year Or Until Contract Expires. • The Provider Should Aid Plm With Any Assistance Requested Until The Project Is Completed, Within The Warranty Period And After Sales Period. • The Provider Should Guarantee Replacement Or Repair Within Warranty Period And Within Agreed Period. If Out Of Warranty Period, The Provider Must Assist Plm For A New Contract And Costing Estimate. Workmanship The Work Throughout Shall Be Executed In The Best And Thorough Manner That Is Under The Direction Of And To The Satisfaction Of The Plm Icto Vp Or Chief Which Shall Have The Power To Reject Any Work And Materials Which, In The Plm Icto Vp Or Chief Judgment, Are Not In Full Accordance Therewith. The Provider Shall Fulfill The Required Services By Using Technical Knowledge That Is In Accordance With The Best-accepted Professional And Industry Standards. The Provider Shall Exercise All Reasonable Skill, Care, And Diligence In The Discharge Of The Duties Agreed To Be Performed And Shall Work In The Best Interest Of Plm. Scope Of Work • One (1) Supply, Installation, Commissioning And Testing Of Ip Pabx Service Including All Essential Parts As May Be Required For Optimal Operations Of The Telephone Service: Pertaining But Not Limited To Network, Security, Hosting, Technical Support, And Any Other Requirements Not Foreseen And Specified. • Installation Of Applications (if Applicable) Relevant To Optimal Operation The Service, Its Inclusions, Security, Quality Performance Requirements, And Service Level Agreement. • Training For The Optimal Operation Of The Service And Inclusions: Including But Not Limited To Specialized Applications, Security, Initial Troubleshooting, And Management. • All The Equipment, Technology, Training, License, And Knowledge Transfer Necessary For The Service Shall Be Retained To Plm. Qualification Requirements • The Specifications Provided Therein Are Minimum Requirements And The Provider May Be Considered To Offer Higher Specifications If It Will Fit On The Required Abc. • The Provider Shall Be Involved In The System Integration Business With At Least 10 Years Of Experience In The Philippines. Must Attach Proof. • The Provider Must Have At Least Two (2) Certified Engineers (at Least Any Ip-pabx Service-related Certification) Who Are Currently Employed In The Contractor’s Company Trained And Have Relevant Certifications For The Service And Inclusions. Must Attach Proof. • The Provider Must Have 24/7 Technical Support In Any Case Of Any Failure For Fast Restoration. • • The Provider Shall Complete The Delivery And Installation Of Work Within Forty-five (45) Calendar Days From The Date Of The Ntp. An Official Project Kickoff Shall Be Called By The Assigned Project Manager For The Provider To Present How It Intends To Implement The Project. The Project Kickoff Shall Be Done Five Days After Receipt Of The Ntp. • Current And Valid Certification From The Principal, Product Manufacturer, Or Authorized Product Distributor, Stating That The Provider Is An Authorized Partner Or Reseller. Failure To Include The Certification In The Eligibility Envelope Is A Ground For Disqualification Of The Provider. • Certification From The Principal Or Product Manufacturer That, End Of Support Of The Proposed Solution Is At Least Within The Three (3) Years Warranty Upon Project Acceptance. • During Contract Implementation, The Provider Must Ensure That It Remains An Authorized Distributor, Reseller, Or Partner For The Maintenance Of The Proposed Solution. If The Service Provider Is Unable To Maintain Its Distributor, Reseller Or Partner Agreement With The Manufacturer/principal Or Distributor, Whichever Is Applicable, This May Serve As A Ground/reason For The Termination Of Its Contract With Plm. • The Subcontractors (if Any) Must Be Certified By The Provider And Must Have At Least 3years Of Service Under The Provider (must Show Proof) • The Plm Shall Meet, Orient And Coordinate With The Winning Contractor Its Rules And Regulations During The Actual Implementation Of The Contract. The Completion Schedule Provided Shall Be Considered Extended Under The Following: Delays Caused By Force Majeure Or Fortuitous Events. In All Cases, The Period Or Number Of Days Of Extension Shall Be Agreed Upon With The Pamantasan Ng Lungsod Ng Maynila In Writing. Claims For Time Extension Of The Contract Period Due To Force Majeure Shall Be Subject To Approval By The Pamantasan Ng Lungsod Ng Maynila. Site Visit • The Provider Is Required To Visit The Site And Satisfy Itself As To Local Conditions And Facilities That May Affect Its Work. The Provider’s Representative/s Will Be Deemed To Have Done This Before Preparing The Proposal, And Subsequent Claims On The Ground Of Inadequate Or Inaccurate Information May Be Given Due Course. Payment Schedule/ Milestones • Payment Shall Be Subject To Applicable Taxes. • Payment Shall Start From The Date Of Ntp For Mobilization. (15% Of Abc) • Upon Complete Delivery Of Equipment. (45% Of Abc) • The Provider Must Also Submit A Signed End-user Acceptance Certificate For The Final Payment If The Product Is Already Installed, Configured, Tested Working, End-user Training, And Implemented To Plm. (40% Of Abc) • The Winning Provider Must Submit The Following Documents Prior To Signing Of Plm Of The Certificate Of Acceptance And Processing Of Payment: Sales Invoice/statement Of Account. License/certificate (if Applicable). Inventory Report/endorsement Of Property; And Pso/twg/end-user Inspection Of Delivery Receipts Other Considerations Provider Shall Provide Project Documentation Such As But Not Limited To: 1. User Manuals Or Guide (ip Phone Configuration, Etc.) 2. Updated Escalation Level Guide With Contact Persons 3. Proof Of License Entitlement And Warranty. 4.
