Description Name Of Project : Supply Of All The Needed Chemicals, Tools, Equipment Apparatus And Accessories, Labor, Technical Supervision And Expertise In Conducting Monthly Comprehensive Pest Control Services, From Basement To 8th Floor, Including Roof Deck, Offices, Comfort Rooms, Hallways, Lobby, Stairs, Cabinets, Drainage, Parking Area And Outside Perimeter Of The Palacio Del Gobernador Building, Corner A. Soriano Avenue And General Luna Street, Intramuros, Manila, Within The Following Scope Of Work. Scope Of Work: 1. Monthly Structural Pest Control Program, By Residual Spraying Of All Wooden Parts To Control Termites And Wood Borers And Prevent Their Re-infestation. 2. Bi-monthly General Pest Abatement Maintenance Program (gpamp), For The Control Of Cockroaches, Mosquitoes, Bugs, Flies, And Other Flying And Crawling Insects. 3. Bi-monthly Rat Abatement Maintenance Program (ramp)- Covers The Control Of All Species Of Rodents Either By The Use Of Exclusive Patented Devices (mechanical) And/or By The Installation Of Slow Or Fast Acting Raticides (chemical) Or Combination Of Both. 4. Once A Month Fogging And Misting At Perimeter This Is Designed To Knock Down All Flying Insects Which Cannot Be Reached By Spraying Like Drainage, Canals, Ceiling Areas And Dark Places. Note: Total Lot Area : 36,400.8 Sq. Meter Terms And Conditions: A. Payment For The Preventive Control Treatment, Shall Be Twelve Equal Monthly Installments To Be Executed Every First Week Of The Month Upon Presentation Of The Original Service Report Duly Acknowledge By The Building Administration Office And The Tenants Representative, Attached To The Invoice Or Statement Of Accounts. B. Payment Shall Be Processed In Accordance With Accounting And Auditing Procedure. No Payment Will Be Released Without The Issuance Of Bir Registered Official Receipt. C. That Only Materials/chemical Approved Authorized By Dost And Fpa Shall Be Used. D. That Materials/chemicals To Be Used Shall Bear The Original Label And Shall Be Checked By The Building Administration Office Prior To Application. E. That Materials/chemicals That Will Directly Affect Human Health Shall Not Be Used. F. The Contract Shall Be A Period Of One (1) Year Location : Palacio Del Gobernador Cond. Corp., A.
