Certificate Of Guarantee That The Service Response Time (physical Presence) Of Service Engineer Or Technician Is Within 24 Hours Upon Notification; Including Weekends And Holidays; 6. Certificate Of Guarantee Issued By The Bidder That In Case Of Breakdown And On-going Repair, The Bidder Shall Provide A Service Unit With Similar Functionality; 7. List Of Completed Similar Brand And Similar Functionality Local Installations By The Winning Bidder In At Least Two (2) Hospitals Or Health Or Research Facilities Excluding Nkti With Contact Person And Phone Numbers; 8. Certificate Of Satisfactory Equipment Performance Within The Last Three (3) Years From At Least Two (2) Local Or Domestic Clients Issued And Signed By The Authorized Officer Of The Head Of Agency Except Nkti With Letterhead And Contact Number; 9. For Former Nkti Suppliers, The Bidder Shall Submit Notarized Sworn Statement That Their Average Performance Evaluation Within The Past Five (5) Years Is At Least Very Satisfactory And There Is No Negative Feedback Or Issues Related To The Installation, And Testing And Commissioning, As Well As Post Acceptance Services Including The Consummated Warranty, Post Warranty Preventive And Corrective Maintenance Of The Supplied Equipment. 10. Certificate Of Guarantee That The Bidder Shall Conduct Orientation And Training For Equipment Operation By A Qualified Trainer Certified By The Principal Or Manufacturer With The Machine For End Users With Issuance Of Applications Training Certification; 11. In The Event That The Original Equipment Manufacturer (oem) Will Be Different From The Principal, The Bidder/supplier Shall Submit A Notarized Document Linking The Relationship Between The Two (2) As Well As The Bidder On Record. 12. Certificate Of Guarantee That The Bidder Shall Provide Physical Demonstration Unit (same Unit/model Offered In The Bid) Within A Non-extendable Five (5) Calendar Days Upon Receipt Of Notification.
