Authorized Representative's Proper Authorization (i.e., In Case Of Partnership, Corporation, Cooperative, Or Joint Venture – Original Or Certified True Copy Of Duly Notarized Secretary’s Certificate, Board/partnership Resolution, Or Special Power Of Attorney, Whichever Is Applicable, Designating The Entity’s Authorized Representative Or In Case Of Sole Proprietorship – Original Or Certified True Copy Of Duly Notarized Special Power Of Attorney For Authorized Representative) Note: 1. Since This Document Is Attached To The Bidder's Original Bid, Representatives Must Keep Their Own Original Or Certified True Copy Of Such Document To Be Presented During Bid Submission. Note: 2.
