Valid Philgeps Registration Number/organization Id/philgeps Certificate Of Registration (red Or Platinum Membership); X 3. Latest Income Or Business Tax Return (for Abcs Above ₱500,000.00); And X 4. Notarized Omnibus Sworn Statement Using Gppb-prescribed Format, Accompanied With Proof Of Authorization, If Applicable (for Abcs Above ₱50,000.00). Proof Of Authorization Shall Be A Duly Notarized Secretary’s Certificate, Board/partnership Resolution, Or Special Power Of Attorney, Whichever Is Applicable, In Case Of A Corporation, Partnership, Or Cooperative, Or Joint Venture; Or A Duly Notarized Special Power Of Attorney In Case Of A Sole Proprietorship, Giving Full Power And Authority To Its Officer To Sign The Oss And Do Acts To Represent The Bidder. Price Quotation Will Be Valid For One Hundred And Twenty (120) Calendar Days From Date Of Opening Of Sealed Bids. 6. The Price Offered Shall Not Be Subject To Any Increase For Whatever Reason Including In Cases Of Devaluation/inflation During The Entire Duration Of The Contract. Iv. Award Of Contract The Bac-twg Of Mlgudds Shall Recommend The Award Of Contract To The Bidder With The Lowest Calculated Responsive Bid (lcrb). V. Performance Security 1. To Guarantee The Faithful Performance Of The Successful Bidder Of Its Obligation Under The Contract, It Shall Post A Performance Security Within Ten (10) Calendar Days From The Receipt Of Notice Of Award (noa), Which Shall Answer For Any Liability Arising From The Performance Of The Contract. 2. The Performance Security Shall Be Denominated In Philippine Pesos And Posted In Favor Of The Procuring Entity In An Amount Not Less Than The Percentage Of The Total Contract Price. 3. The Performance Security Posted In Favor Of The Procuring Entity Shall Be Forfeited In The Event It Is Established That The Contractor Is In Default In Any Of Its Obligations Under The Contract. Vi.
