Description Conduct Of Health And Wellness Program (annual Physical Examination, Vaccination, And Drug Test) For Dot Employees For Cy 2024 Ii. Purpose/objective The Department Of Tourism, As Part Of Its Health And Wellness Programs Will Be Conducting Various Medical Programs, Projects, And Activities For Dot Employees In The Regional Office Which Includes Annual Physical Examination (ape), Anti-flu And Anti-pneumonia Vaccination, And Mandatory Drug Test. This Is To Ensure The Health And Wellness Of The Dot Workforce Considering That The Physical Well-being Of Employees Has A Significant Impact On The Efficiency And Effectiveness Of Public Service Delivery, And Also To Reduce The Percentage Of Employees With Sickness Attributable To Health And Wellness Interventions, Exclusive Of Declared Existing Condition And Comorbidities Of Employee. Iii. Legal Basis Based On The Section 31 Of Chapter V, Subtitle A, Title I Of Book V Of The Executive Order No. 292, Each Department Or Agency Must Uphold And Promote Employees’ Health And Wellness To Ensure Maximum Efficiency And Productivity In The Workplace. Iv. Eligibility Requirements 1. The Service Provider Must Be A Department Of Health (doh)-recognized Service Provider With Vaccines That Are Registered With The Doh And Food And Drug Administration (fda). 2. Must At Least Be Five (5) Years In Operation. 3. Must Have Catered Similar Projects In The Past With At Least Three (3) Government And/or Private Companies (corporate Client), With Certification Of At Least Satisfactory Performance. 4. Must Be Willing To Provide Services On A Send-bill Arrangement. 5. Amenable To Date Changes. V. Details Of Scope Of Work And Deliverables 1. The Provider Must Ensure The Provision Of Doh Accredited Specimen Collectors, Who Are Medical Technologies And/or Nurses On The Actual Conduct Of Onsite Testing; 2. The Provider Must Submit Photocopies Of Specimen Collectors’ Prc Licenses And Doh Certificates To The Dot Medical Clinic A Week Before The Implementation Of The Drug Test; 3. Implementation Date/s Shall Be Set As Agreed By The Provider And The Dot Iv-a Regional Office For Twenty-three (23) Employees (plantilla And Job Orders) 4. Employees Who Will Not Be Tested Onsite Shall Have Their Drug Test Done At The Provider’s Clinic/laboratory, The Schedule Shall Be Arranged By Dot And The Provider; 5. The Following Are The Expected Outputs Of The Provider: • Employee Individual Results • Tabulated Summary Of Results • Final Report On The Consolidated Results Of The 2024 Mandatory Drug Testing Program The Abovementioned Outputs Must Be Submitted In Hard And Soft Copies Within Two (2) Weeks After The Conduct Of The Activity Before The Release Of The Certificate Of Completion; 6. The Provider Must Provide All Necessary Forms, Equipment, Supplies, And Paraphernalia During The Conduct Of The Said Program; 7. The Provider Is Responsible For The Disposal Of All Used Materials And Articles. These Biological Wastes Should Be Disposed Of According To The Procedures Prescribed By The Doh Health Care Waste Management Manual; 8. The Provider Shall Coordinate With The Dot Ro Before, During And After The Screening. **please Refer To The Main Document For A More Detailed Information** Vi. Approved Budget For Contract The Approved Budget For Contract For The Conduct Of Health And Wellness Program (annual Physical Examination, Vaccination, And Drug Test) For Dot Employees For Cy 2024 Is Two Hundred Twenty Four Thousand Two Hundred Twenty Seven Pesos (php 224,227.00) Vii. Legal Documents 1. Valid Business/mayor’s Permit 2. Philgeps Registration 3. Omnibus Sworn Statement Viii. Terms Of Payment • Payment Shall Be Made To The Supplier Not Less Than Thirty (30) Working Days After The Project Has Been Completed • The Payment Shall Be On A Send Bill Arrangement, Which Shall Be Settled Upon Submission Of The Statement Of Account With A Breakdown Of Expenses And Other Payment Documents After The Project Has Been Completed • Payment Shall Be Based On Actual Expenses Incurred But Not To Exceed Two Hundred Twenty Four Thousand Two Hundred Twenty Seven Pesos (php 224,227.00) For The Total Actual Cost Of Services Rendered Inclusive Of Vat And Other Taxes.
