Submission Of Proposals Including The Following Documentary Requirements Addressed To Dr. Melvin B. Carlos Through The Bac Secretariat Shall Be On Or Before April 26, 2024, At The Physical Address Or Through The Email Address Below: Mayor’s/business Permit*; Philgeps Registration Number*; Notarized Omnibus Sworn Statement (oss); Conformity With The Terms Of Reference; Professional License And Curriculum Vitae Of The Required Team Members; And The Service Provider Must Meet And Demonstrate The Minimum Qualifications: Accredited By The Philippine Accreditation Bureau (pab) Or Any Accreditation Body To Provide Qms Certification To Iso 9001:2015 At Least 5 Years Of Experience In Conducting Iso Qms Audits/certification In Private Organizations; And At Least 5 Years Of Experience In Iso Qms Government Certification Projects; At Least 1 Project (iso Audits/certification) Similar/relevant To The Mandates Of Pcaarrd.
