The Services Outlined In This Request For Proposal (rfp) Are Primarily Funded Through The California Equitable Recovery Initiative (ceri), As A Part Of The Centers For Disease Control And Preventions National Initiative To Address Covid-19 Health Disparities Among Populations At High-risk And Underserved (cdc-rfa-ot21-2103) And May Be Supplemented By The California Strengthening Public Health Infrastructure Federal Funding. the Covid-19 Pandemic Was A Stark Reminder Of How Long-standing Inequities Due To Systemic Barriers And Historic Injustices Lead To Unacceptable Disparities Across Health Outcomes. While The Covid-19 State Of Emergency Has Ended, It Is Important To Recognize That The Disproportionate Impacts It Had Across Populations Will Continue To Be Felt For Years To Come. california Has Aimed To Strengthen Equity Infrastructure - At The State, Regional, And Local Levels - To Support A More Coordinated Approach To How We Advance Health Equity As A Statewide Public Health Priority. Cdph Has Been Working In Collaboration With Local Health Jurisdictions (lhjs), Community-based Organizations (cbos), And Other Key Partners To Development And Implement A State Health Equity Plan (shep). the Shep Will Serve As The Shared Equity Strategy And Implementation Plan For Lets Get Healthy California, The State Health Improvement Plan (ship). The Efforts Aim To Collectively Advance Health Equity And Improve Community Health, Especially For Populations Experiencing Significant Disparities Across Health Outcomes.
