I. Specific Deliverables: 1. The Prospective Bidders Shall Bid For The Provision Of Catering Services For The Packed Am Snacks, Lunch And Pm Snacks For The Supply And Delivery Of Packed Meal For The Conduct Of Face-to-face Workshop On The Review And Validation Of Criteria For The Gawad Kampilan And Division Excellence Awards As Follows: 1.1 Packed Meals For Packed Am Snacks, Lunch And Pm Snacks For The Supply And Delivery Of Packed Meal For The Conduct Of Face-to-face Workshop On The Review And Validation Of Criteria For The Gawad Kampilan And Division Excellence Awards, Consisting Of At Least The Following Foods: A. Chicken Dish B. Fish / Seafood Dish C. Vegetables Dish D. Soup E. Dessert F. Rice G. Juice H. Pasta H. The Caterer Shall Provide The Set Of Plates, Spoon And Fork, Flatware And All Necessary Dining Materials Good For Above-mentioned Participants With Reasonable Allowance. The Caterer Shall Provide Long Table For Food Set-up. 3. The Caterer Shall Provide Service Staff With Their Proper Uniform. Ii. Other Terms And Conditions 1. All Other Incidental Expenses In The Performance Of This Agreement/package Shall Be Covered By The Provider Unless Previously Agreed To By The Deped Rizal. 2. Payment Shall Be Made Upon Caterer’s Issuance Of Statement Of Account Subject To Applicable Taxes. 3. All Bid Prices Shall Be Considered As Fixed Prices, And Therefore Not Subject To Price Escalation During Contract Implementation. 4. The Caterer Shall Strictly Observe And Implemented Iatf Safety Protocols Against Covid-19. Iii. General Conditions 1. All Quotations Must Be Computerized / Typewritten In The Company’s Letterhead. 2. The Following Documents Shall Be Submitted And Attached Before Issuance Of Notice Of Award : • Mayor’s Or Business Permit; • Philgeps Registration Number; And • Omnibus Sworn Statement 3. The Processing Of Payment Shall Be Prepared Upon Receipt Of The Supplier’s Statement Of Account, Subject To The Deduction Of Applicable Taxes.
