Description The Deped Baguio Teachers Camp Through It's Bids And Awards Committee (bac) Is Now Inviting Interested Suppliers To Quote On The Following Items: 1 125 Pax Medical And Physical Examination Package: Diagnostic And Screening Procedure, Interpretation Of Results/consultation And Recommendations "1. Blood Chemistry To Include: - Liver Enzymes (sgpt, Sgot, And Adt) - Lipid Profile (total Cholesterol, Triglycerides, Hdl, Ldl, Vldl) - Blood Urea Nitrogen (bun) - Creatinine - Blood Uric Acid (bua)" "2. 12 Lead Electrocardiogram (ecg) With Reading And Interpretation 3. Whole Abdominal Ultrasound 4. Complete Blood Count With Platelet Count 5. Hba1c 6. Blood Typing 7. Routine Urinalysis 8. Drug Testing Thc And Methamphetamines 9. Conduct The Following Services On-site (btc- Benitez Hall) On These Dates:" "a. Medical Examination (april 16 And 17, 2024 6:30am-2:00pm)" " 1. Interpretation Of Results/consultation On Laboratory Tests Result/s 5. Recommendation" Terms And Conditions 1 The Service Provider Must Be A Reputable Laboratory/clinic/facility, Duly Accredited By The Department Of Health, Preferably A Local Supplier To Ensure And Guaranteeaccurat, Error-free And Reliable Laboratory Test Results. Attach Certified Copy Of Department Of Health (doh) Accreditation As A Diagnostic/laboratory Facility For The Current Year. 2 The Service Provider Must Provide An On-site Service On The Specified Dates, Have A Readily Available And Fully Functional Equipment (i.e Mobile X-ray Machine, One (1) Mobile Ultrasound Machine And Two (2) Ecg Machines For The Entirety Of The Ampe Period. 3 The Service Provider Shall Provide Capable And Experienced Health Care Professionals (nurses/medical/rediological Technicians) And/or Administrative Staff Who Shall Conduct The On-site Diagnostic And Other Ancillary Medical Procedures/examination Services Of Btc Employees Including Registration, Taking Of Anthropometric Measurements Adn Vital Signs: A) At Least Three (3) Physicians (male/female)assigned For The Physical Examination And Interpretation Of Results Of The Ampe During Consultancy; "b) At Least One (1) Physician For The Visual Examination (promotion/selling Of Prescription/fashion Eyeglasses Including Suplements Is Strictly Prohibited);" "c) At Least Two (2) Medical Technicians Assigned For Blood Extraction; D) At Least One (1) Registered Nurse Assigned To Obtain Vital Signs; E) At Least Two (2) Ecg Technicians (male/female)" "f) At Least Two (2) Personnel (male/female) Assigned For Collecting Urine Specimens; G) At Least One (1) Ultrasound Technician; And H) At Least Two (2) Adminsitrative Staffs For The Registration And/or Filling-up Of Necessary Forms (waiver Or Consent Forms) And To Attend To Other Concerns Of Employees During The Conduct Of The Projects." 4 The Service Provider Must Submit The Complete List Of Their Personnel (name Of Physician, Pathologist, Sonologist, Nurses, Medical Technologist, Radiologist And Radiologic Technician, Staff) Who Will Conduct The Ampe. 5 The Service Provider Must Submit Certified Copies Of Their Valid And Updated Prc License Of The Applicable Personnel. 6 The Service Provider Must Disinfect Their Equipment After Every Use (i.e Ultrasound Transducer, Ecg Electrodes, Bp Apparatus, Weighing Scale, Etc.) 7 Provide The Following Equipment/materials/services: A) Disposable Gloves And Face Mask For Technicians Who Will Use Various Equipment: B) Appropriate Garbage Bags, Disposable Bins Especially For Hazardous Wastes; C) Extension Cords; D) Urine Specimen Cups; E) Dininfectant/alcohol; F) Tissue Paper, Etc 8 The Service Provider Must Secure The Written Waiver Of Rights From The Employees Of The Right To The Confidentiality Of Information Between The Physician And Patient For Procedures And Examinations Undertaken In Behalf Of The Btc. The Written Waiver Shall Include The Authorization In The Health Profile Of The Employees. 9 The Service Provider Shall Submit And Coordinate With The Personnel Unit The Ampe Results Within 10 Calendar Days (cds) From The Time Of The Conduct Of The Ampe Of Btc: A) Summary Of Medical Examination Report Of The Employees (1 Hard Copy Certified By The Service Provider Physician And Soft Copy) B) 3 Sets Hard Copy Of The Medical Examination Report Of Btc Employees (individual) Signed And Sealed, 3 Copies: 1 For The Employees, 1 For The 201 File And 1 For Reference Of The Management. C) Summary Of Consulatation Report. All Physicians Findings And Patients History After The Consultation And/or Physicians Findings After The Consultation Shall Be Submitted Within 1 Week After The Consultation Date For 201 File Record, Signed And Sealed. 10 All Incurred Related Expenses (meals, Transportation, Lodging Etc.) For The Staff/personnel Of The Service Provider For The Duration Of The Ampe Shall Be Shouldered By The Servcie Provider. Terms Of Payment 1 Payment Shall Be Made After The Completion Of The Services, Submission Of Result/findings/recommendations And Other Requirements (i.e Soa, Ampe Results). 2 The Service Provider Shall Forward The Statement Of Account And Other Pertinent Documents Required For The Services Rendered Within Two (2) Weeks From The Last Scheduled Testing/screening Date. 3 After The Completion, Validation, And Review, Btc Shall Settle All Obligations Incurred For The Services, Subject To The Existing Budgeting, Accountingand Auditing Laws, Rules And Regulations. Other Requirements For The Project 1 The Bidder Must Be Of Good Standing And Has At Least Satisfactory Performance With Respect To It's Latest Completed Projects. Attach/submit List Of All Completed Projects (at Least 5 Years Prior To The Deadline For Submission Of Bid) For Reference. 2 Subcontracting Is Not Allowed. **********nothing Follows*********** Instructions: 1 Accomplish This Rfq Correctly And Accurately. 2 Do Not Alter The Contents Of This Form In Any Way. 3 Technical Specifications Are Mandatory. 4 Failure To Follow These Instructions Will Disqualify Your Entire Quotation. Terms And Conditions 1. Bidder Shall Provide Correct And Accurate Information Required In This Form. Bidding Procedures Will Be Conducted In Accordance With The Provisions Of The Implementing Rules And Regulations (irr) Of The Republic Act 9184 And The Deped Customized Agency Procurement Manual (cam). 2. Price Quotation/s Must Be Valid For A Period Of Forty-five (45) Calendar Days From The Date Of Submission. 3. Price Quotation/s, To Be Denominated In Philippine Peso Shall Include All Taxes, Duties And/or Levies Payable. 4. Quotations Exceeding The Approved Budget For The Contract Shall Be Rejected. 5. Any Interlineations, Erasures Or Overwriting Shall Be Valid Only If They Are Signed Or Initiated By You Or Any Of Your Duly Authorized Representative/s. 6. It Is The Intent Of The Purchaser To Evaluate The Bid/quotation On: [ ] Lot Basis, Hence Only Complete Bids Are Accepted, No Partial Bids Allowed. Award Shall Be Made To The Bid/ Quotation Or Combination Of Quotations Resulting In The Total Lowest Calculated And Responsive Bid/quotation (for Good And Infrastructure) Or, The Highest Rated Offer (for Consulting Services) Which Complies With The Technical Specifications And Other Terms And Conditions Stated In The Contract / Po. The Item/s Shall Be Delivered Within The Terms Specified In The Approved Purchase Order (po)/ Notice To Proceed (ntp). Payment Shall Be Made Immediately Upon Receipt Of The Goods (complete) And Or Completion Of Service/s (as Specified In The Terms And Conditions In The Contract/po)/ Demand For Payment And Submission Of The Required Supporting Documents If Any. 8. Representatives From The Property And Supply Section, Commission On Audit (coa), And Accounting Section Shall Have The Right To Inspect And / Or Test The Goods To Confirm Its Compliance To The Technical Specifications. 9. Liquidated Damages Equivalent To One Tenth Of One Percent (0.001%) Of The Value Of The Goods Not Delivered Within The Prescribed Delivery Period Shall Be Imposed Per Day Of Delay. The Baguio Teachers Camp Shall Rescind The Contract / Po Once The Cumulative Amount Of Liquidated Damages Reaches Ten Percent (10%) Of The Amount Of The Contract, Without Prejudice To Other Courses Of Action And Remedies Open To It. 10.the Btc-bac Reserves The Right To Accept Or Reject Any And All Bid/s, To Annul The Bid Opening Process, And To Reject All Bids At Any Time Prior To Contract Award, Without Thereby Incurring Any Liability To The Affected Bidder/s. 11. Quotations, Including Documentary Requirements, Received After The Deadline Shall Not Be Accepted.
