Description Invitation To Bid For The Supply And Delivery Of Four Units (4) Hammer Mill And Four (4) Units Corn Sheller 1. The Department Of Agriculture - Regional Field Office 1 (da-rfo 1), Through The National Expenditure Program Fy 2025 Intends To Apply The Sum Of Two Million Four Hundred Sixty Thousand Pesos (php2,460,000.00) With Project Identification Number Da-rfo-1-2025-goods-015 Being The Approved Budget For The Contract (abc) To Payments Under The Contract For Each Lot, To Wit: Project Title Abc (php) Lot 1: Supply And Delivery Of Four (4) Units Hammer Mill 1,060,000.00 Lot 2: Supply And Delivery Of Four (4) Units Corn Sheller 1,400,000.00 Total 2,460,000.00 2. The Da-rfo 1 Now Invites Bids For The Early Procurement Activity (epa) Of The Above Procurement Project, In Accordance With The Provisions Under Appendix 31 Of The 2016 Revised Implementing Rules And Regulations (irr) Of Republic Act (ra) No. 9184 For The Above-mentioned Procurement Project With The Following Details And Minimum Specifications, To Wit: Lot No. Particulars/descriptions 1 Hammer Mill Swinging-type/fixed Type Input Capacity: 500 Kg/hr Material For Input Hopper And Milling Assembly Must Be Made Of Non Corrosive Material With Sorter Output: Cracked Corn; Corn Grits; Corn Bran With Trailing Wheels And Handle With On/off Button For Emergency Purposes Primemover: 3hp Electric Motor Single Phase Other Requirements: I. To Be Submitted During Bid Opening: A. Valid Amtec Test Report B. Valid Permit To Operate Issued By Bafe And Certificate Of Namdac Accreditation Issued By Pcaf; Certificate Of Conformity Of The Hammer Mill And Engine C. Certificate Of Company Owned/affiliated/authorized Service Center Within The Region And Service Center Must Have A Permit To Operate Issued By Bafe; Affiliated Service Center Should Be Supported With A Notarized Servicing Agreement. D. Notarized Affidavit Of Undertaking After Sales Service That Supplier Must Render Within 72 Hours Upon Receipt Of Notice From The Recipients/procuring Entity E. Certificate Of Availability Spare Parts Of The Brand/model Being Offered Within The Region To Be Issued By The Distributor/manufacturer/affiliated/authorized Service Center F. Notarized Certificate Of 100% Stock Allocation To Be Submitted During Bid Opening Issued By The Manufacturer Specifying The Name Of Project To Da Rfo 1 G. Certificate To Conduct On-site Technical Briefing For Operators On Operation, Maintenance, And/or Repair Of The Supplied Goods By The Supplier Ii. To Be Submitted/verified During Post-qualification A. Shall Present At Least 1 Unit Of The Hammermill And Its Primemover And Certificate Of Conformity Of Engine Iii. To Be Submitted/inspected/verified During Delivery/after Delivery A. With Warranty Of 1 Year On Parts Of Equipment And 2 Years For Services To Be Reckoned From Date Of Delivery B. With Conduction Sticker Bearing Da Logo (4"x 4"), Labeled With "corn- Rfo 1" Below The Logo C. With Metal Plate Indicating The Company Name And Active Office Landline/mobile Numbers For Easy Access Of Farmers Client D. The Technical And Financial Requirement Each In Separate Envelopes, Shall Be Enclosed In A Mother Envelope With The Same Sealing And Markings. Non-compliance With Index Tabbings Shall Not Be A Ground For Outright Disqualification Or Declaration Of Ineligibility. The Improper Index Tabbings Must Be Duly Acknowledged By The Bidder/representative And Be Subject To The Bid Evaluation And Post-qualification Of The Technical Working Group (twg) As To Their Substance. The Technical Specifications Of The Project Must Be Duly Signed/countersigned And Sealed By A Licensed Agricultural And Biosystems Engineer In Accordance With Republic Act 8559 Or Republic Act 10915; E. The Award Of Contract For Procurement Projects Undertaken Through Epa May Be Made Only Upon The Following Conditions And Shall Be In Accordance With Item 7 Of Appendix 31 Of The 2016 Revised Irr Of Ra 9184: F.1.
