Description 1. One (1) Year Provision Of Janitorial Services At Cugman Elementary School Rendering 8 Hrs Daily 2. The Agency Shall Provide One Utility Personnel Who Is - One (1) Female - Must Be 25 Years Old And Above - Must Have Sss,phic,hdmf Account - Must Have Updated Sss,phic,hdmf Remittances - Of Good Moral Character And Without Criminal Or Police Record - Physically And Mentally Fit To Work - Able To Work With Less Supervision - Do Errand: Submit Documents To Different Offices And Pay Bills - Maintain Cleanliness In The Office, School Clinic, School Library & Other Facilities - Other Related Works That May Be Required 3. The Agency And Utility Personnel Shall Perform The Terms And Condition Stipulated In The Contract. 4. All Billing By The Agency Shall Be Payable On A Monthly Basis Upon Receipt Of The Client Of The Agency's Statement Of Account With All Supporting Documents 5. Proof Of Remittances ( Sss,phic And Hdmf ) Of The Utility Shall Be Submitted In The School Finance Staff.
