Description Provision Of Security Services Procurement Terms And Conditions: Approved Budgetary Requirement: (note: 27,000.00 Per Guard Per Month For Cy 2025 X 2 Guards X 12 Months, Inclusive Of Tax Obligations And Statutory Payables E.g. Sss, Pag-ibig And Philhealth) No. Of Hours/duty Per Day: 12 Hours Per Guard Terms Of Service: January 1, 2025 To December 31, 2025 Place Of Assignment, Work Schedule, And Scope Of Services: Cda Region Viii Extension Office, New Bus Terminal Compound, Brgy. 91, Abucay, Tac. City A.) The Agency Shall Assign One (1) Security Guard Per Shift (or 2 Guards Shifting Schedule) To Man The Perimeters Of The Office As Well As All The Properties, Equipment And Vehicles. B.) The Guards Shall Take Turns In Shifting At Cda Office Everyday To Man The Premises. C.) Ensure The Safety Of Cda Official, Personnel And Its Clients/guests From Harassment, Threat, Or Intimidation Within Cda Premises. D.) The Contractor Shall Make Available, At Its Own Expense, Such Number Of Relievers As May Be Necessary, Who Are Ready To Take Over The Duty Schedules Of Those Regularly Assigned Guards Who Either Report Late Or Are Absent For The Day At No Cost To Cda. E.) Strict Observance Of Cda Policies, Rules And Regulations On Security F.) Secure The Cda Properties (supplies And Equipments) From Theft, Damage And Arson. G.) Other Duties And Functions: - Protect Entire Work Area Occupied By Cda, Its Employees And Clients Against Unauthorized Intrusion, Internal Or External Commotion - Check, Entertain And Assist Clients Or Visitors Entering Cda Premises. - Strictly Maintains Official Logbook Or Record Of All Incoming And Outgoing Persons, Vehicles, Movements Of Cda Properties, Among Others. - Records Employees’ Time In And Out. - After Office Hours, Conduct Routine Patrol Or Inspection Of The Building And Report Unusual Activities Or Threat To Cda Officer-in-charge - Ensure That All Doors And Windows Of Cda Building Are Secured And All Electrical Connections And Lights Are Turned Off. Before The Start Of Office Hours, Inspect All Areas For Any Sign Of Forced Entry. H.) Failure Of The Supplier To Deliver Goods And Render Services Under The Contract Within The Specified Delivery Schedule Shall Be Liable For Damages For The Delay Pursuant To Sec. 3 Annex D Of The 2016 Revised Irr Of Ra 9184. I.) The Head Of The Agency Reserves The Right To Reject Any And All Bids, Declare A Failure Of Bidding Or Not Award The Contract Per Sec. 41 Of The Of The 2016 Revised Irr Of Ra 9184. Qualifications Of Service Provider A. Should Have At Least Five (5) Years Of Providing Security Services To A Private And/or Government Agency. B. Must Be A Member Of The Philippine Association Of Detective And Protective Agency Operators (padpao) In Good And Active Standing. C. Must Be Duly Registered With Security And Exchange Commission, Department Of Trade And Industry, Or Cooperative Development Authority. D. Must Be Registered With Sss, Pag-ibig, And Philhealth. E. Must Be Registered With Bir. F. With Valid Business Permit. G. Must Have Security Service Office Within Tacloban City H. Certified Photocopy Of Security Guard's License To Carry Firearms To Operate Should Be Submitted To The Office. I. Has A Pnp Sosia License/permit. Provision Of Supplies And Materials By The Service Provider To Their Security Guards - The Security Agency Shall Also Provide The Security Guards With Basic Security Kits (baton, Handcuffs, Flashlight, Raincoat & Handheld Radio). - The 2 Security Guards Shall Have A Valid Security License. - The Security Agency Shall Provide The Security Guards To Carry Licensed Pistol With Basic Load Ammunition. It Must Be Present & Be Used (if Deemed Necessary) By The Guard-on-duty. What To Submit (documentary Requirements) 1.) The Security Agency Shall Submit A Copy Of Their Updated Security Guard's Security License. 2.) The Security Agency Shall Submit A Copy Of Its License To Operate Approved By Camp- Crame. 3.) Copy Of 2024 Mayor's/business Permit 4.) Omnibus Sworn Statement 5.) Quotation Form 6.) Copy Of Philgeps Registration/ Philgeps Certificate 7.) Copy Of Dole Registration And Clearance 8.) Copy Of Income Or Business Tax Returns 9.) Cost Breakdown Duly Signed By The Supplier’s Authorized Representative" 10.) Company Profile Terms Of Payment: 1.
